布魯氏桿菌病為全球范圍內(nèi)最常見的人畜共患病之一[1],常通過感染的組織、體液、牛奶或乳制品傳染給人類,屬自然疫源性疾病[2],國內(nèi)多分布于西北、東北等畜牧區(qū)[3]。布魯氏桿菌性脊柱炎好發(fā)于腰椎,胸腰段次之[4]。腰背痛是最常見的臨床表現(xiàn)[5],由于該病臨床表現(xiàn)多樣且無特異性,與其他脊柱疾病高度相似,臨床醫(yī)生對此病認(rèn)識(shí)不足常被誤診、誤治,加之發(fā)病率逐年上升[6-10],嚴(yán)重影響患者的日?;顒?dòng)及工作,因此對于本病磁共振成像(magnetic resonance imaging,MRI)表現(xiàn)的掌握顯得十分重要。本文對我院在2015-2017年的65例布魯氏桿菌性脊柱炎患者的MRI圖像進(jìn)行回顧性分析,這些病例均經(jīng)臨床確診且影像學(xué)資料完整,探討MRI檢查尤其是精確頻率反轉(zhuǎn)恢復(fù)(spectral presaturation attenuated inversion recovery,SPAIR)序列在診斷布魯氏桿菌性脊柱炎中的價(jià)值,以期為臨床診斷和治療提供指導(dǎo)。
回顧性分析2015年1月至2017年4月我院的65例布魯氏桿菌性脊柱炎患者的MRI圖像資料,其布病玻片凝集實(shí)驗(yàn)或虎紅平板凝集實(shí)驗(yàn)呈陽性,男性48例,女性17例,年齡27~79歲,平均年齡55.5歲,本組中生活在牧區(qū)者58例、食用過不潔肉制品者5例,市民2例。本研究所有患者下腰痛,腰痛1~6個(gè)月不等,49例患者同時(shí)發(fā)熱、乏力,56例首診。
血常規(guī)顯示6例白細(xì)胞正常,血沉加快36例,布魯氏桿菌血清凝集實(shí)驗(yàn)結(jié)果均為陽性,滴度>1∶200。
根據(jù)我國疾病預(yù)防和控制中心診斷標(biāo)準(zhǔn),流行病學(xué)接觸史,實(shí)驗(yàn)室結(jié)果支持,有相應(yīng)影像學(xué)表現(xiàn),排除其他感染性疾病及腫瘤,臨床確診為布魯氏桿菌性脊柱炎[11]。
采用Philips Achiva 1.5 T超導(dǎo)型磁共振掃描器。頸椎使用表面線圈,胸、腰段使用脊柱專用線圈。掃描序列:快速自旋回波序列(turbo spin echo,TSE)序列T1WI (TR/TE=450 ms/15 ms)、TSE序列T2WI (TR/TE=3500 ms/120 ms)和SPAIR序列T2WI (TR/TE=3000 ms/100 ms),層厚3.0~4.0 mm,層間距1.0 mm。增強(qiáng)掃描:患者靜注釓雙胺,注射劑量15 ml,流率2 ml/s。經(jīng)肘靜脈注射后進(jìn)行脊柱軸位、矢狀位和冠狀位掃描。圖像分析:圖像處理采用GE公司SUN Workstation 4.3工作站的Functool 5.4.07軟件。由2名副高以上放射科診斷醫(yī)師觀察所有患者影像資料,對布魯氏桿菌性脊柱炎MRI表現(xiàn)的特點(diǎn)進(jìn)行記錄和總結(jié)。
采用SPSS 17.0統(tǒng)計(jì)學(xué)分析軟件,對65例患者的常規(guī)序列T1WI和T2WI的各個(gè)征象計(jì)數(shù)結(jié)果進(jìn)行χ2檢驗(yàn),P<0.05為差異具有統(tǒng)計(jì)學(xué)意義。對其中同時(shí)行常規(guī)序列、SPAIR序列掃描的45例布魯氏桿菌性脊柱炎患者的每個(gè)征象的各序列計(jì)數(shù)結(jié)果分別進(jìn)行χ2檢驗(yàn),P<0.05為差異具有統(tǒng)計(jì)學(xué)意義。
65例布魯氏桿菌性脊柱炎患者中,52例有2個(gè)椎體受累,13例有2個(gè)以上椎體受累,總計(jì)144個(gè)病椎。腰椎120個(gè),骶椎3個(gè),胸椎12個(gè),頸椎9個(gè)。
41例病椎形態(tài)正常,僅出現(xiàn)骨髓水腫者58例,骨質(zhì)楔變者24例,部分椎體邊緣骨贅形成。病變多見于椎體前、中部,T1WI呈較均勻低信號(hào),T2WI呈高低混雜信號(hào),SPAIR序列脂肪高信號(hào)變?yōu)榈托盘?hào),使高信號(hào)病灶更明顯。53例椎間隙狹窄,12例椎間隙正常,46例椎間盤信號(hào)異常。58例椎旁軟組織增厚,其中椎旁膿腫58例,椎旁膿腫合并硬膜外膿腫17例,膿腫為圓形或紡錘形呈長T1、長T2 信號(hào),且范圍較局限,不超過2個(gè)椎體,毗鄰腰大肌腫脹。增強(qiáng)掃描病灶呈中等強(qiáng)度均勻強(qiáng)化,膿腫邊緣環(huán)形強(qiáng)化,SPAIR序列呈高信號(hào)。28例患者出現(xiàn)伴隨征象,其中23例椎小關(guān)節(jié)炎,5例前縱韌帶鈣化。65例患者中,T1WI對于骨髓水腫顯示更加明顯,T2WI對于椎旁膿腫的顯示更加明顯,二者檢查結(jié)果差異均具有統(tǒng)計(jì)學(xué)意義(P<0.05)。骨質(zhì)破壞、楔變、椎間隙狹窄及硬膜外膿腫以上病變T1WI與T2WI差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。僅45例行SPAIR序列及增強(qiáng)掃描檢查。在這45例患者中,骨髓水腫、椎旁膿腫、硬膜外膿腫以上病變于T2WI像及SPAIR序列均呈高信號(hào)改變,SPAIR序列對于骨髓水腫顯示更加明顯,二者檢查結(jié)果差異均具有統(tǒng)計(jì)學(xué)意義(P<0.05)。骨質(zhì)破壞、楔變、椎間隙狹窄、椎旁膿腫及硬膜外膿腫以上病變MRI常規(guī)序列及SPAIR序列差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。典型病例見圖1~5。布魯氏桿菌性脊柱炎的常規(guī)MRI序列的檢查結(jié)果見表1,各表現(xiàn)在T2WI及SPAIR序列中的統(tǒng)計(jì)數(shù)據(jù)見表2。

布魯氏桿菌經(jīng)破損皮膚、消化道黏膜或呼吸道侵入人體后,到達(dá)附近淋巴結(jié)生長繁殖,之后被巨噬細(xì)胞殺滅,未能被殺滅者則繼續(xù)生長繁殖形成感染灶,最終沖破淋巴結(jié)屏障進(jìn)入血液形成菌血癥,繼之侵犯網(wǎng)狀內(nèi)皮系統(tǒng)。布魯氏桿菌性脊柱炎主要以滲出、增生、肉芽腫3種病理改變交替發(fā)生[12]。布魯氏桿菌性脊柱炎患者通常以三聯(lián)征為主要癥狀,即腰背痛、波浪熱和病畜接觸史,與國外研究基本一致[13-14],部分伴有脊柱強(qiáng)直、肢體麻木、關(guān)節(jié)痛、肝、脾和淋巴結(jié)腫大等。實(shí)驗(yàn)室檢查:血清凝集實(shí)驗(yàn)、補(bǔ)體結(jié)合實(shí)驗(yàn)呈陽性、抗人球蛋白實(shí)驗(yàn)呈陽性均對診斷該病有一定幫助,確診需檢查出致病菌。本文發(fā)現(xiàn)65例患者中單核細(xì)胞增高37例,其原因可能為該菌主要影響富含單核吞噬細(xì)胞的肝、脾、淋巴結(jié)和骨髓等,與文獻(xiàn)一致[15],所以可將單核細(xì)胞升高作為診斷依據(jù)之一。本研究中大部分患者的紅細(xì)胞沉降率(erythrocyte sedimentation rate,ESR)有不同程度的升高,與國外學(xué)者[13]研究亦基本一致。
3.2.1 椎體改變
本組布魯氏桿菌脊柱炎患者發(fā)生于腰椎最多見,L4、L5常見,頸椎、胸椎少見,與文獻(xiàn)[16-17]報(bào)道基本一致,筆者認(rèn)為可能與腰椎活動(dòng)度較大且承重較多有關(guān)。本組中同時(shí)累及連續(xù)兩個(gè)相鄰椎體和2個(gè)以上椎體占100%,與國外研究基本一致[18],其中同時(shí)累積相鄰椎體可能是由于椎體動(dòng)脈發(fā)出血管分支同時(shí)為相鄰兩椎體供血[19]。本組有1例跳躍性多發(fā)椎體病灶,有學(xué)者[20]認(rèn)為可能也與布魯氏桿菌血液傳播有關(guān)。布魯氏桿菌性脊柱炎可導(dǎo)致椎體骨質(zhì)破壞,Thammaroj等[21-22]指出該病累及椎體形態(tài)基本正常且破壞程度較輕。本組研究與文獻(xiàn)報(bào)道一致,僅有24例(24/65,36.9%)椎體前緣不同程度楔變,其余椎體未見塌陷、壓縮變扁、脊柱后凸畸形。骨質(zhì)破壞常表現(xiàn)為不規(guī)則蟲蝕樣,以椎體前部對吻面為主,僅有26例(26/65,40.0%)骨質(zhì)破壞明顯,累及椎體中后部,椎弓根未見受累,39例(39/65,60.0%)未見明顯骨質(zhì)破壞,究其原因可能是骨質(zhì)增生修復(fù)速度大于骨質(zhì)破壞速度,故患者整個(gè)椎體形態(tài)未見明顯變化,無脊柱后突畸形[23]。受累椎體呈長T1信號(hào),部分信號(hào)不均勻,T2WI相出現(xiàn)高、等、低混雜信號(hào),可能因病變不同階段出現(xiàn)骨質(zhì)增生、硬化所致。SPAIR序列因去除脂肪組織的干擾,使炎性滲出少量液體呈明顯高信號(hào)[24](圖1),信號(hào)欠均勻,故此序列對于病灶較敏感[25]。增強(qiáng)掃描顯示病椎明顯不均勻強(qiáng)化(圖2)。
3.2.2 椎間盤改變
本組中有53例(53/65,81.5%)椎間隙變窄,明顯變窄者少見,可能與纖維組織形成有關(guān)。椎間盤信號(hào)無明顯變化,其中大多數(shù)是等T1、等T2信號(hào),僅有5例椎間盤在T2WI及SPAIR序列呈高信號(hào),Eker等[26]認(rèn)為布魯氏桿菌性脊柱炎早期,病變始于血運(yùn)豐富的椎體上終板前緣,逐漸向椎體及間盤進(jìn)展,因此對于早期病變患者,SPAIR序列是必不可少的。
3.2.3 椎旁軟組織改變
隨著疾病進(jìn)展,病變逐漸侵及椎旁軟組織和椎管,形成椎旁膿腫。Boyaci等[27]發(fā)現(xiàn),布魯氏桿菌性脊柱炎患者中硬膜外膿腫發(fā)生率極低。本組中58例(58/65,89.2%)有椎旁軟組織增厚,毗鄰腰大肌腫脹,表現(xiàn)為不規(guī)則欠均勻長T1、長T2信號(hào),SPAIR序列呈混雜高信號(hào),范圍較局限,邊界模糊,內(nèi)含大小不等、環(huán)形強(qiáng)化膿腔,壁厚薄不均(圖3、4)。筆者發(fā)現(xiàn)腰椎較頸椎椎管外軟組織受累明顯,可能與其椎管周圍空間狹窄限制了炎癥的擴(kuò)散有關(guān)[28]。本組中17例(17/65,26.2%)形成硬膜外膿腫,與文獻(xiàn)[28]報(bào)道較接近,但一些學(xué)者[29-30]認(rèn)為其發(fā)生率僅為1~2/10000,考慮原因?yàn)樗占±幱诓煌〕屉A段、布魯氏桿菌出現(xiàn)變異。膿腫多見于硬膜囊前間隙內(nèi),呈長T1、長T2信號(hào),SPAIR序列呈高信號(hào),增強(qiáng)掃描后膿腔壁明顯強(qiáng)化,厚薄不均,膿腔不強(qiáng)化。
3.2.4 伴隨征象
椎小關(guān)節(jié)炎:常見于相鄰椎體上下關(guān)節(jié)突關(guān)節(jié),關(guān)節(jié)面毛糙,小關(guān)節(jié)間隙變窄或融合,呈長T1、長T2信號(hào),部分呈混雜T2信號(hào),SPAIR序列表現(xiàn)為不均勻稍高信號(hào)(圖5)。本組中23例(23/65,35.4%)有椎小關(guān)節(jié)炎。韌帶鈣化:累及韌帶時(shí)常表現(xiàn)為條索狀等或長T1、短T2信號(hào)[31],前后縱韌帶內(nèi)呈點(diǎn)片狀稍長T2信號(hào)。5例前縱韌帶鈣化。

筆者認(rèn)為,布魯氏桿菌性脊柱炎在臨床上較為少見,又因誤診率較高,布魯氏桿菌病嚴(yán)重影響患者的社會(huì)和工作生活質(zhì)量[32],故對其進(jìn)行準(zhǔn)確及時(shí)的診斷至關(guān)重要。MRI對組織內(nèi)水、蛋白質(zhì)含量改變十分敏感,能對椎體骨質(zhì)破壞、椎間盤破壞、椎旁及椎管內(nèi)外膿腫等情況很好地顯示[25,33],MRI在疾病早期階段能更敏感地檢測椎體和周圍軟組織信號(hào)的改變。SPAIR序列是MRI檢查中十分重要的技術(shù),合理利用脂肪抑制序列技術(shù)既可以明顯提高圖像質(zhì)量,減少運(yùn)動(dòng)偽影、化學(xué)位移偽影或其他相關(guān)偽影,提高病變檢出率,又能為鑒別診斷提供重要信息[34]。本組所研究的布魯氏桿菌性脊柱炎患者,椎體破壞輕微且局限,椎間隙稍狹窄或正常,同樣椎旁軟組織改變亦較局限,膿腫范圍小,一般僅累及上下椎體,未見累及其臨近器官。在臨床工作中,應(yīng)與結(jié)核性脊柱炎鑒別,其主要鑒別點(diǎn)是骨質(zhì)破壞嚴(yán)重,且多出現(xiàn)脊柱后凸畸形,不但伴較大范圍椎旁膿腫,而且累及鄰近器官。然而布魯氏桿菌性脊柱炎多見于農(nóng)牧區(qū),多有家畜接觸史,所以在該類地區(qū)就診的患者應(yīng)高度懷疑布魯氏桿菌性脊柱炎,并行一些必要檢查,如布魯氏桿菌凝聚實(shí)驗(yàn)、補(bǔ)體結(jié)合實(shí)驗(yàn)等,以便及早確診。國內(nèi)學(xué)者李華[35]和石冬等[36]分析該病患者M(jìn)RI資料,并指出布魯氏桿菌性脊柱炎患者沒有椎旁膿腫形成。與文獻(xiàn)不同的是,本研究發(fā)現(xiàn)有58例布魯氏桿菌性脊柱炎患者出現(xiàn)椎旁膿腫,只是范圍大小不同,但與早期脊柱結(jié)核的膿腫不易鑒別。因此對于確診該病,需結(jié)合布魯氏桿菌凝集實(shí)驗(yàn)、補(bǔ)體結(jié)合實(shí)驗(yàn)或血培養(yǎng)。本研究結(jié)果顯示,布魯氏桿菌性脊柱炎具有一定特征性的影像學(xué)表現(xiàn),MRI是目前最為有效、敏感的檢查手段,其中SPAIR序列有著非常重要的作用,尤其對于脂肪含量較多的中老年患者,能清楚地顯示磁共振常規(guī)掃描中發(fā)現(xiàn)不了的病變,所以在中老年人有相關(guān)臨床癥狀時(shí)SPAIR序列非常必要,為影像科診斷醫(yī)生出具正確診斷提供技術(shù)支持,建議在中老年人腰椎磁共振掃描時(shí)常規(guī)增加SPAIR序列。而對于該病的確診需根據(jù)臨床表現(xiàn)、病史、MRI圖像及實(shí)驗(yàn)室檢查等綜合判斷。總之,布魯氏桿菌性脊柱炎雖少見,但臨床醫(yī)生需提高對其重視程度,避免誤診、漏診,從而提高其診斷水平。
