The diagnostic value analysis of low-dose multi-slice helical CT scan in 83 children with acute appendicitis

[Abstract] Objective To explore the diagnostic value of low-dose multi-slice helical CT scan in children with acute appendicitis. Methods A total of 165 patients admitted to our hospital from December 2016 to December 2020 were randomly divided into observation group(n=83) and control group(n=82). The observation group used Brilliance nano low-dose 128-slice helical CT scan, and the control group used Brilliance nano conventional-dose 128-slice helical CT scan.The scan dose, effective milliampere second, the positive detection rate between the two groups were observed and compared. Results The scan dose of the observation group was (8.29±2.38)mGy,? lower than that of the control group (19.33±4.51)mGy. The effective milliampere second (86.73±23.08)mAs in the observation group was lower than that (162.02±47.71) mAs in the control group. The difference between the two groups was statistically significant(P<0.05). The positive detection rate of the observation group was 92.77% (77/83), and the positive detection rate of pathological results was 96.39% (80/83). There was no significant difference between the two methods(P>0.05).The positive detection rate of the control group was 86.59%(71/82), which was lower than that of pathological results (96.34%, 79/82). The difference between the two methods was statistically significant(P<0.05). Conclusion Low-dose Multi-slice helical CT scan is effective in the diagnosis of acute appendicitis in children. It can observe the lesions in multiple directions and multiple planes,effectively improve the sensitivity and specificity of clinical diagnosis, and has high safety. It is worthy of popularization and application.

兒童急性闌尾炎屬于小兒外科常見急腹癥,年齡≤5歲的兒童,急性闌尾炎發(fā)病率占所有闌尾病例的15%~24%[1-2]?;純涸缙跓o典型臨床癥狀,診斷漏診及誤診率較高[3-5],而且往往手術(shù)和病理組織學(xué)檢查結(jié)果之間的差異很大[6]。因此,早發(fā)現(xiàn)、早診斷、早治療,具有重要臨床意義。臨床對于兒童急性闌尾炎的診斷,常規(guī)CT掃描診斷靈敏度及特異度較高;但常規(guī)射線計(jì)量對患兒的輻射較大[7-8]。多層螺旋CT低劑量掃描技術(shù)憑借強(qiáng)大的后處理功能及低輻射等優(yōu)點(diǎn),從不同角度觀察組織器官解剖結(jié)構(gòu),在兒童急性闌尾炎的診斷中被廣泛重視應(yīng)用[9]。我院2016年12月至2020年12月共收治兒童急性闌尾炎患兒165例,采用多層螺旋CT低劑量掃描技術(shù)進(jìn)行診斷,現(xiàn)報(bào)道如下。
