面神經(jīng)監(jiān)測(cè)在聽(tīng)神經(jīng)瘤術(shù)中面神經(jīng)保護(hù)作用的研究

【摘要】 目的:探討面神經(jīng)監(jiān)測(cè)在聽(tīng)神經(jīng)瘤術(shù)中面神經(jīng)解剖及功能保留的應(yīng)用。方法:回顧性對(duì)比分析本科室2007年1月-2012年1月普通聽(tīng)神經(jīng)瘤手術(shù)面神經(jīng)的解剖及功能保留的25例患者(未監(jiān)測(cè)組)與2012年2月-2016年2月術(shù)中行監(jiān)測(cè)的27例患者(監(jiān)測(cè)組)的聽(tīng)神經(jīng)瘤的臨床資料。結(jié)果:術(shù)后,未監(jiān)測(cè)組的腫瘤全切少于監(jiān)測(cè)組,差異有統(tǒng)計(jì)學(xué)意義( 字2=3.107,P=0.078)。未監(jiān)測(cè)組死亡1例,監(jiān)測(cè)組無(wú)死亡病例,兩組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05)。術(shù)后,監(jiān)測(cè)組僅12例面神經(jīng)解剖保留,少于監(jiān)測(cè)組的24例,差異有統(tǒng)計(jì)學(xué)意義( 字2=10.188,P=0.001)。兩組術(shù)后的面神經(jīng)功能分級(jí)比較,差異有統(tǒng)計(jì)學(xué)意義(u=482,P=0.004)。結(jié)論:聽(tīng)神經(jīng)瘤術(shù)中面神經(jīng)監(jiān)測(cè)可顯著提高術(shù)后面神經(jīng)解剖及功能的保留率。

【Abstract】 Objective:To explore the application of facial nerve monitoring for the facial nerve anatomy and function protection in acoustic neuromas surgery.Method:The clinical data of acoustic neuromas operation with non monitering 25 cases(non monitoring group)from January 2007 to January 2012 and 27 cases using facial nerve monitoring(monitoring group) from February 2012 to February 2016 were collected and analyzed by retrospective.Result:After operation,the total tumor resection in the non monitoring group was less than that in the monitoring group,the difference was statistically significant( 字2=3.107,P=0.078).There was 1 death in the non monitoring group,and there was no death in the monitoring group,there was no significant difference between the two groups(P>0.05).After the operation,only 12 cases of facial nerve were preserved in the monitoring group,less than 24 cases in the monitoring group,the difference was statistically significant( 字2=10.188,P=0.001).There were significant differences in facial nerve function classification between the two groups after operation(u=482,P=0.004).Conclusion:Facial nerve electrophysiological monitoring can improve the preservation rate of facial nerve anatomy and function in acoustic neuroma operation.

聽(tīng)神經(jīng)瘤占顱內(nèi)腫瘤的8%~10%[1],是常見(jiàn)的顱內(nèi)腫瘤之一,早期聽(tīng)神經(jīng)瘤治療完全依賴手術(shù)切除,治療也以全切腫瘤為目標(biāo),隨著顯微外科技術(shù)的發(fā)展,聽(tīng)神經(jīng)瘤手術(shù)死亡率已不足0.5%[2],但術(shù)后面癱嚴(yán)重影響患者生活質(zhì)量,尤其是在大型聽(tīng)神經(jīng)瘤術(shù)后[3]。既往手術(shù)面神經(jīng)的辨認(rèn)主要靠術(shù)中的解剖標(biāo)志和術(shù)者的經(jīng)驗(yàn)與猜測(cè)。因此面神經(jīng)保留率不高,直徑大于3.0 cm時(shí),1年面神經(jīng)保留率在27%~58%[4],一項(xiàng)meta分析表明,大型聽(tīng)神經(jīng)瘤術(shù)后面神經(jīng)解剖保留率為88.8%,功能優(yōu)良率僅為62.9%[5]。因此功能保留逐漸成為治療的首要目標(biāo),本研究擬采用術(shù)中面神經(jīng)的電生理監(jiān)測(cè)技術(shù)(intraoperative facial nerve monitoring,IFNM)探討該方法對(duì)聽(tīng)神經(jīng)瘤術(shù)中面神經(jīng)定位、解剖與功能保護(hù)的意義?,F(xiàn)報(bào)告如下。

1 資料與方法

1.1 一般資料 回顧性對(duì)比分析本科室2007年

1月-2012年1月普通聽(tīng)神經(jīng)瘤手術(shù)面神經(jīng)的解剖及功能保留的25例患者(未監(jiān)測(cè)組)與2012年

2月-2016年2月術(shù)中行監(jiān)測(cè)的27例患者(監(jiān)測(cè)組)的聽(tīng)神經(jīng)瘤的臨床資料。其中男21例,女31例,年齡32~66歲,平均(44.5±8.4)歲,病程2個(gè)月~20年,聽(tīng)力下降30例,聽(tīng)力喪失12例,伴頭痛、行走不穩(wěn)12例,術(shù)前全部行頭顱MRI平掃及增強(qiáng),左側(cè)腫瘤為30例,右側(cè)腫瘤為22例,腫瘤直徑為1.8~5.5 cm,患者術(shù)前術(shù)后面神經(jīng)功能評(píng)估采用H-B分級(jí)。兩組患者的一般資料比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),具有可比性。endprint

1.2 手術(shù)方法 患者均采用丙泊芬+吸入麻醉,肌松藥物從開(kāi)始切開(kāi)硬膜就停止用藥。所有患者均采用乙狀竇后入路。選擇乳突后直切口,長(zhǎng)約6 cm,骨窗大小約2 cm×3 cm。暴露橫竇與乙狀竇交匯及乙狀竇后緣。以乙狀竇為中心弧形切開(kāi)硬腦膜后先釋放腦橋小腦池或延髓小腦池內(nèi)腦脊液,使腦組織充分塌陷。沿小腦半球外側(cè)達(dá)小腦腦橋角區(qū),暴露腫瘤后,先電生理監(jiān)測(cè)主動(dòng)電刺激(0.1 mA)腫瘤背側(cè),查看腫瘤背側(cè)是否存在面神經(jīng),沒(méi)有面神經(jīng)則先切開(kāi)背側(cè)行囊內(nèi)減壓,視具體磨鉆磨除內(nèi)聽(tīng)道后壁,手術(shù)過(guò)程中密切觀察動(dòng)態(tài)肌電圖,口、眼輪匝肌出現(xiàn)電活動(dòng)時(shí)及時(shí)反饋給手術(shù)醫(yī)師,注意保護(hù)面神經(jīng),直至肌電圖正常。無(wú)法辨別的組織是否為面神經(jīng)時(shí),需以電刺激確認(rèn),先以小電流0.1 mA開(kāi)始,并逐漸增大至0.5 mA,依據(jù)面肌肌電圖勾畫(huà)出面神經(jīng)纖維的走行方向,腫瘤切除在橋小腦角池、橋前池等蛛網(wǎng)膜界面內(nèi)進(jìn)行。使用美國(guó)尼高力(Nicolet Endeavor CRl6)通道的監(jiān)護(hù)儀,監(jiān)測(cè)患者的面神經(jīng)自由肌電和間斷刺激器電流誘發(fā)電位。記錄電極(針型電極)分別刺入雙側(cè)眼輪匝肌和口輪匝肌,以貼膜固定電生理監(jiān)測(cè)系統(tǒng)。手術(shù)過(guò)程中對(duì)疑為神經(jīng)的組織以刺激探頭刺激,刺激量由小到大,結(jié)合監(jiān)測(cè)術(shù)中持續(xù)自發(fā)面神經(jīng)自由肌電圖,準(zhǔn)確向術(shù)者反饋各神經(jīng)信息。肌電圖的分析時(shí)間100 ms,靈敏度35 μV,濾波20~1500 Hz;未監(jiān)測(cè)組即未使用相關(guān)監(jiān)測(cè)設(shè)備,僅僅通過(guò)術(shù)中的經(jīng)驗(yàn)、靠術(shù)中的解剖標(biāo)志和術(shù)者的猜測(cè)判斷面神經(jīng)的位置。

1.3 觀察指標(biāo) (1)術(shù)后第一天復(fù)查MRI平掃及增強(qiáng),了解腫瘤的切除程度。(2)術(shù)后,觀察兩組患者臨床表現(xiàn)(如肢體偏癱、失語(yǔ)、死亡等)。(3)面神經(jīng)功能評(píng)定:對(duì)所有患者出院時(shí)面神經(jīng)功能評(píng)定,按House-Brackmann(H-B)標(biāo)準(zhǔn)評(píng)分確定面神經(jīng)功能:Ⅰ級(jí)為功能正常;Ⅱ級(jí)為輕度功能障礙;Ⅲ級(jí)為中度功能障礙;Ⅳ級(jí)為重度功能障礙;Ⅴ級(jí)為嚴(yán)重功能障礙;Ⅵ級(jí)為完全麻。

1.4 統(tǒng)計(jì)學(xué)處理 對(duì)數(shù)據(jù)的分析以及處理均使用軟件SPSS 17.0進(jìn)行,手術(shù)后的腫瘤全切率、面神經(jīng)解剖保留率均為計(jì)數(shù)資料,以率(%)表示,組間比較采用 字2檢驗(yàn),面部神經(jīng)功能分級(jí)為等級(jí)資料,采用秩和檢驗(yàn),P<0.05為差異有統(tǒng)計(jì)學(xué)意義。

2 結(jié)果

術(shù)后,未監(jiān)測(cè)組的腫瘤全切少于監(jiān)測(cè)組,差異有統(tǒng)計(jì)學(xué)意義( 字2=3.107,P=0.078),見(jiàn)表1。未監(jiān)測(cè)組死亡1例,監(jiān)測(cè)組無(wú)死亡病例,兩組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05)。術(shù)后,監(jiān)測(cè)組僅12例面神經(jīng)解剖保留,少于監(jiān)測(cè)組的24例,差異有統(tǒng)計(jì)學(xué)意義( 字2=10.188,P=0.001)。兩組術(shù)后的面神經(jīng)功能分級(jí)比較,差異有統(tǒng)計(jì)學(xué)意義(u=482,P=0.004),見(jiàn)表2。典型病例見(jiàn)圖1。

3 討論

聽(tīng)神經(jīng)瘤是顱內(nèi)常見(jiàn)的良性腫瘤,由于其生長(zhǎng)緩慢,且聽(tīng)力下降癥狀不易察覺(jué),故腫瘤發(fā)現(xiàn)時(shí)往往巨大。根據(jù)聽(tīng)神經(jīng)瘤多學(xué)科協(xié)作診療中國(guó)專家共識(shí)的建議[6],除完全位于內(nèi)聽(tīng)道內(nèi)的小腫瘤可考慮觀察、隨訪外。突入橋小腦腳區(qū)的腫瘤,除無(wú)法耐受手術(shù)建議立體定向放射外科治療,余建議行手術(shù)切除治療。隨著顯微外科技術(shù)進(jìn)步及精準(zhǔn)醫(yī)學(xué)的發(fā)展,聽(tīng)神經(jīng)瘤手術(shù)腫瘤切除死亡率已不足0.5%[2],但術(shù)后面癱成為影響患者生活質(zhì)量的最重要因素。在聽(tīng)神經(jīng)瘤中,正常的面神經(jīng)因腫瘤的擠壓、推擠后形態(tài)及位置發(fā)生改變,早期手術(shù)面神經(jīng)的辨認(rèn)和保護(hù)主要靠術(shù)中的解剖標(biāo)志、術(shù)者的經(jīng)驗(yàn)等,準(zhǔn)確率不佳,面神經(jīng)保留率不佳。面神經(jīng)的解剖和功能是術(shù)中面神經(jīng)監(jiān)測(cè)的解剖基礎(chǔ),而手術(shù)特別是在腫瘤暴露和切除的過(guò)程中,除面神經(jīng)受到牽連刺激可產(chǎn)生誘發(fā)自由肌電電位外,術(shù)中可使用低電量電流(如0.1 mA)主動(dòng)電刺激可疑為面神經(jīng)的組織產(chǎn)生的神經(jīng)肌肉復(fù)合動(dòng)作電位(CMAP)可分辨是否為面神經(jīng),并根據(jù)肌電圖分布規(guī)律描繪出面神經(jīng)的走行,為面神經(jīng)的解剖保留提供了電生理依據(jù)。術(shù)中電生理監(jiān)測(cè),提示面神經(jīng)的可能位置。同時(shí)主動(dòng)的電刺激驗(yàn)證是否為面神經(jīng)。若可疑為面神經(jīng),則術(shù)中止血時(shí)雙極電凝使用時(shí)盡量小功率,盡量減少雙極的使用,且牽拉暴露分離腫瘤時(shí)輕柔操作,最大限度地保留面神經(jīng)功能[7]。

其次需掌握橋小腦角區(qū)的顯微解剖知識(shí),尤其是面神經(jīng)的正常解剖位置。面神經(jīng)與腫瘤的關(guān)系較為恒定,絕大多數(shù)文獻(xiàn)報(bào)道面神經(jīng)位于腫瘤的前方腹側(cè)[8-10]。文獻(xiàn)[11]研究表明,大部分聽(tīng)神經(jīng)瘤位于內(nèi)聽(tīng)道內(nèi)的蛛網(wǎng)膜下腔,隨著腫瘤的生長(zhǎng)向內(nèi)側(cè)推擠覆蓋于腫瘤表面的蛛網(wǎng)膜與橋小腦池內(nèi)的橋腦中腦外側(cè)膜形成兩層膜性結(jié)構(gòu),內(nèi)為小腦腦橋角池的重要血管和神經(jīng)結(jié)構(gòu),但瘤組織始終位于橋腦中腦外側(cè)膜之外[12],這是腫瘤能夠全切除的解剖基礎(chǔ)。

文獻(xiàn)[13]首次報(bào)道橋小腦角區(qū)腫瘤使用術(shù)中電生理監(jiān)測(cè)技術(shù)以來(lái),此監(jiān)測(cè)技術(shù)已成為聽(tīng)神經(jīng)瘤手術(shù)的常規(guī)技術(shù)。監(jiān)測(cè)有:牽拉或操作誘發(fā)的自由肌電圖及探針間斷小電流刺激面神經(jīng)可疑區(qū)產(chǎn)生的刺激肌電圖。記錄面肌的復(fù)合動(dòng)作電位,然后通過(guò)計(jì)算機(jī)輔助,根據(jù)曲線判斷面神經(jīng)的位置及功能。動(dòng)態(tài)持續(xù)監(jiān)測(cè)神經(jīng)肌電活動(dòng),及時(shí)反饋術(shù)中的危險(xiǎn)操作。在牽拉或擠壓面神經(jīng)時(shí)可誘發(fā)出小的間斷的自發(fā)性面肌電圖,監(jiān)護(hù)儀發(fā)出警報(bào),提醒術(shù)者。雖然面神經(jīng)位于腫瘤背側(cè)的比例小于1%[14],但仍然需在切開(kāi)腫瘤前先用面神經(jīng)監(jiān)測(cè)儀探測(cè),確認(rèn)無(wú)面神經(jīng)后再切開(kāi)腫瘤背側(cè)的包膜進(jìn)行腫瘤內(nèi)減壓,通常采用超聲吸引(CUSA)吸除腫瘤減壓,這樣可減少腫瘤的牽拉引起面神經(jīng)的損傷。等腫瘤體積縮小、橋小腦角池空間增大后,再通過(guò)磨開(kāi)內(nèi)聽(tīng)道后壁尋找面神經(jīng)遠(yuǎn)端或于腦干端尋找面神經(jīng)近端,或兩者相結(jié)合的方式。當(dāng)分離瘤壁牽拉面神經(jīng)時(shí),連續(xù)監(jiān)測(cè)可誘發(fā)出肌電圖,表現(xiàn)為連續(xù)收縮的波形,結(jié)合小電流(通常0.1 mA)電刺激可對(duì)面神經(jīng)的走行進(jìn)行定位。在操作時(shí)牽拉腫瘤或可疑的神經(jīng)組織時(shí)需停止操作,尤其需等面神經(jīng)警報(bào)消失后,才能進(jìn)行下一步操作。術(shù)中監(jiān)測(cè)的重點(diǎn)應(yīng)在切除內(nèi)聽(tīng)道內(nèi)或近腦干端腫瘤時(shí)。當(dāng)面神經(jīng)與腫瘤包膜在顯微鏡下難以分辨時(shí),面神經(jīng)刺激肌電圖監(jiān)測(cè)是尋找和確認(rèn)面神經(jīng)的唯—手段。腫瘤切除完畢,通過(guò)0.1 mA刺激肌電圖還可以證實(shí)面神經(jīng)結(jié)構(gòu)和功能完整與否[15]。Samii等[16]報(bào)道通過(guò)術(shù)中電生理監(jiān)測(cè)可描繪出面神經(jīng)走向,能有效保護(hù)面神經(jīng)功能。Yamakami等[17]通過(guò)術(shù)中面神經(jīng)監(jiān)測(cè)在大型聽(tīng)神經(jīng)鞘瘤手術(shù)中其面神經(jīng)解剖保留率達(dá)到92%,其中面神經(jīng)Ⅰ、Ⅱ級(jí)功能約達(dá)84%。而本組52例聽(tīng)神經(jīng)瘤經(jīng)中,前25例未行術(shù)中面神經(jīng)監(jiān)測(cè)指導(dǎo),解剖保留率為48.0%,而使用術(shù)中面神經(jīng)監(jiān)測(cè)面神經(jīng)解剖保留率提高到88.9%,而Ⅰ、Ⅱ級(jí)面神經(jīng)功能比例達(dá)70.4%,大大提高面神經(jīng)的解剖及功能保留率。endprint

通過(guò)IFNM應(yīng)用和技術(shù)的進(jìn)步,面神經(jīng)解剖及功能保留已達(dá)到極高水平,但文獻(xiàn)[18]報(bào)道:面神經(jīng)即使解剖保留完整,有些患者術(shù)后仍有不同程度的功能障礙。但文獻(xiàn)[19]報(bào)道有的手術(shù)全程都未能監(jiān)測(cè)到面神經(jīng),但術(shù)后患者面神經(jīng)仍具有一定功能,而術(shù)中可記錄到良好的面神經(jīng)肌電圖像,術(shù)后其面癱未必不重,因此術(shù)中監(jiān)測(cè)雖可提示面神經(jīng),也許在操作及分離切除腫瘤過(guò)程中面神經(jīng)已經(jīng)收到損傷,因此目前在聽(tīng)神經(jīng)瘤中面神經(jīng)保護(hù)研究中仍有很多疑慮,需要進(jìn)一步的研究。

總之,聽(tīng)神經(jīng)瘤手術(shù)時(shí)術(shù)中面神經(jīng)監(jiān)測(cè)可提示面神經(jīng)的位置,根據(jù)肌電圖勾畫(huà)出面神經(jīng)的走行。尤其是在可疑面神經(jīng)組織時(shí),進(jìn)行主動(dòng)電刺激驗(yàn)證及判斷面神經(jīng)的位置,更好的保護(hù)面神經(jīng)的解剖結(jié)構(gòu)及功能。
