早期康復(fù)訓(xùn)練對(duì)早產(chǎn)兒腦損傷綜合征患者生長(zhǎng)發(fā)育及神經(jīng)系統(tǒng)后遺癥的影響

[摘要] 目的 探討早期康復(fù)訓(xùn)練對(duì)早產(chǎn)兒腦損傷綜合征患者生長(zhǎng)發(fā)育及神經(jīng)系統(tǒng)后遺癥的影響。 方法 選取2013年1月~2016年7月腦損傷綜合征早產(chǎn)兒180例,根據(jù)早產(chǎn)兒家長(zhǎng)是否積極參與干預(yù)分為干預(yù)組134例與對(duì)照組46例。對(duì)照組早產(chǎn)兒予以常規(guī)干預(yù),干預(yù)組早產(chǎn)兒在此基礎(chǔ)上予以早期康復(fù)訓(xùn)練,兩組早產(chǎn)兒均干預(yù)24個(gè)月。觀察兩組早產(chǎn)兒干預(yù)前后生長(zhǎng)發(fā)育指標(biāo)的變化,并比較神經(jīng)系統(tǒng)后遺癥情況。 結(jié)果 干預(yù)12個(gè)月和24個(gè)月后,干預(yù)組早產(chǎn)兒身高、體質(zhì)量和智力發(fā)育商水平均明顯高于對(duì)照組(P<0.05);干預(yù)24個(gè)月,干預(yù)組殘留的早產(chǎn)兒神經(jīng)系統(tǒng)后遺癥的發(fā)生率(7.46%)明顯低于對(duì)照組(19.57%)(χ2=4.11,P<0.05)。 結(jié)論 早產(chǎn)兒腦損傷綜合征患者予以早期康復(fù)訓(xùn)練可修復(fù)受損的神經(jīng)細(xì)胞,促進(jìn)其生長(zhǎng)發(fā)育,降低殘留的神經(jīng)系統(tǒng)后遺癥的發(fā)病率,有利于改善患兒的預(yù)后。

[Abstract] Objective To investigate the effect of early rehabilitation training on growth and development and neurological sequelae in patients with premature infants with brain injury syndrome. Methods A total of 180 preterm infants with brain injury syndrome from January 2013 to July 2016 were enrolled. According to whether the parents of premature infants actively participated in the intervention,the patients were divided into the intervention group(n=134) and the control group(n=46). The premature infants in the control group were routinely intervened,and the preterm infants in the intervention group were given early rehabilitation training on the above basis. Both groups of premature infants were intervened for 24 months. The changes of growth and development indicators before and after intervention of the two groups were observed,and the sequelae of the nervous system between two groups were compared. Results After 12 months and 24 months of intervention, the height, body weight and mental development of the preterm infants in the intervention group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05). After 24 months of intervention,the incidence of neurological sequelae in preterm infants (7.46%) was lower than that in the control group(19.57%),and the difference was statistically significant (χ2=4.11,P<0.05). Conclusion Early rehabilitation training for patients with brain injury syndrome in premature infants can repair nerve cell damage, promote the growth and development of premature infants, reduce the incidence of neurological sequelae, and improve the prognosis of children.

腦損傷綜合征是由于患兒在圍生期各種因素引起的非進(jìn)行性腦組織損傷,常導(dǎo)致小兒腦癱、智能低下、癲癇發(fā)作、語(yǔ)言障礙、生長(zhǎng)發(fā)育遲緩、情感障礙和行為異常,是嬰兒期致殘的常見(jiàn)原因,對(duì)社會(huì)和家庭造成沉重的負(fù)擔(dān)[1,2]。隨著醫(yī)學(xué)技術(shù)的進(jìn)展,腦損傷綜合征早產(chǎn)兒的存活率較以往有明顯提高,但其干預(yù)后遺留的神經(jīng)系統(tǒng)方面的并發(fā)癥仍居高不下[3,4]。研究已證實(shí)3歲前兒童的大腦發(fā)育可塑性與代償性最強(qiáng),早期康復(fù)訓(xùn)練可有效提高腦損傷綜合征早產(chǎn)兒的生存率,減少神經(jīng)系統(tǒng)后遺癥的發(fā)生率,但國(guó)內(nèi)在這方面研究報(bào)道較少[5,6]。本研究分析觀察了早產(chǎn)兒腦損傷綜合征患者予以早期康復(fù)訓(xùn)練對(duì)生長(zhǎng)發(fā)育及神經(jīng)系統(tǒng)后遺癥的改善作用,現(xiàn)報(bào)道如下。

1 資料與方法

1.1 一般資料

選取2013年1月~2016年7月期間我院兒科治療腦損傷綜合征早產(chǎn)兒180例。納入標(biāo)準(zhǔn)[7]:生后6 h內(nèi)經(jīng)振幅整合腦電圖(Amplitude-integrated electroencephalogram,aEEG)檢查確診。排除標(biāo)準(zhǔn)[8]:(1)出生時(shí)存在先天性畸形或遺傳方面疾病;(2)新生兒期合并嚴(yán)重疾病。根據(jù)家長(zhǎng)是否積極參與干預(yù)將早產(chǎn)兒分為兩組。兩組早產(chǎn)兒的一般資料方面統(tǒng)計(jì)學(xué)分析比較基本一致(P>0.05)。見(jiàn)表1。

1.2 方法

對(duì)照組早產(chǎn)兒予以健康教育、育兒知識(shí)指導(dǎo)、病情觀察、定期檢查及父母心理干預(yù)等常規(guī)干預(yù)。干預(yù)組早產(chǎn)兒在此基礎(chǔ)上予以早期康復(fù)訓(xùn)練,內(nèi)容:①視覺(jué)刺激:用黑白視覺(jué)訓(xùn)練卡和紅球,每天數(shù)次引逗患兒的注意以刺激視覺(jué)發(fā)育,20 min/次,3次/d;②聽(tīng)覺(jué)刺激:每天聽(tīng)音調(diào)悠揚(yáng)而音色優(yōu)美的樂(lè)曲,20 min/次,3次/d;③觸覺(jué)刺激:通過(guò)皮膚按摩及撫摸肢體等刺激訓(xùn)練觸覺(jué)發(fā)育;④平衡感干預(yù):將患兒抱在懷里搖晃、旋轉(zhuǎn)、側(cè)滾等方式刺激平衡感發(fā)育;⑤被動(dòng)操與康復(fù)訓(xùn)練:予以肢體曲伸,誘導(dǎo)患兒伸手、抓握蹬腿等主動(dòng)和被動(dòng)訓(xùn)練進(jìn)行被動(dòng)操與康復(fù)訓(xùn)練。兩組均干預(yù)24個(gè)月。分析兩組早產(chǎn)兒干預(yù)前后生長(zhǎng)發(fā)育指標(biāo)的變化,并比較神經(jīng)系統(tǒng)后遺癥情況。

1.3 觀察指標(biāo)

1.3.1 生長(zhǎng)發(fā)育指標(biāo)[5]? 包括身高、體質(zhì)量和智力發(fā)育商這3項(xiàng)指標(biāo)評(píng)估早產(chǎn)兒的生長(zhǎng)發(fā)育情況。智力發(fā)育商采用Gesell發(fā)育診斷法量表進(jìn)行評(píng)估,包括語(yǔ)言、精細(xì)運(yùn)動(dòng)、個(gè)人-社交、大運(yùn)動(dòng)、適應(yīng)性等5項(xiàng),分?jǐn)?shù)越高表現(xiàn)智力發(fā)育越好。

1.3.2 神經(jīng)系統(tǒng)后遺癥[1]? 包括聽(tīng)力障礙、語(yǔ)言障礙、智力低下、腦癱和癲癇等。

1.4 統(tǒng)計(jì)學(xué)處理

采用SPSS20.0統(tǒng)計(jì)學(xué)軟件進(jìn)行分析,計(jì)量資料以均數(shù)±標(biāo)準(zhǔn)差(x±s)表示,采用t檢驗(yàn),計(jì)數(shù)資料以[n(%)]表示,采用χ2檢驗(yàn),P<0.05為差異有統(tǒng)計(jì)學(xué)意義。

2 結(jié)果

2.1 兩組早產(chǎn)兒干預(yù)12個(gè)月和24個(gè)月后生長(zhǎng)發(fā)育指標(biāo)的比較

干預(yù)12個(gè)月和24個(gè)月后,干預(yù)組早產(chǎn)兒身高、體質(zhì)量和智力發(fā)育商水平均明顯高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。見(jiàn)表2。

2.2 兩組早產(chǎn)兒殘留的神經(jīng)系統(tǒng)后遺癥的發(fā)生率比較

干預(yù)24個(gè)月,干預(yù)組早產(chǎn)兒殘留的神經(jīng)系統(tǒng)后遺癥的發(fā)生率(7.46%)明顯低于對(duì)照組(19.57%),差異有統(tǒng)計(jì)學(xué)意義(χ2=4.11,P<0.05)。見(jiàn)表3。

3 討論

以往對(duì)早產(chǎn)兒腦損傷綜合征的治療多側(cè)重于早期生命的搶救方面,而忽視了早期康復(fù)鍛煉在患兒預(yù)后改善方面的重要作用。近年來(lái)隨著醫(yī)學(xué)技術(shù)的進(jìn)步,改善早產(chǎn)兒腦損傷治療的預(yù)后,預(yù)防或減少神經(jīng)系統(tǒng)并發(fā)癥的發(fā)生率日益受到臨床關(guān)注[9-12]。嬰幼兒在3歲前正處于大腦系統(tǒng)發(fā)育核心期和快速期,其腦組織系統(tǒng)可塑性與代償性較高,當(dāng)腦損傷患兒的局部大腦細(xì)胞損傷后修復(fù)能力較強(qiáng),其功能可被周?chē)渌?xì)胞的有關(guān)功能所代償,并在大腦受損的相應(yīng)部位進(jìn)行有效橋接、重組和發(fā)育,從而產(chǎn)生良好的神經(jīng)細(xì)胞代償功能,主要表現(xiàn)為大腦樹(shù)突細(xì)胞數(shù)量增多和髓鞘的發(fā)育;另一方面,雖然神經(jīng)細(xì)胞在發(fā)生腦損傷后會(huì)出現(xiàn)神經(jīng)元的凋亡,但這凋亡過(guò)程是一個(gè)可逆的過(guò)程,且持續(xù)時(shí)間相對(duì)較長(zhǎng),在此期間為患兒予以積極有效的行為護(hù)理干預(yù),對(duì)患兒的感官、運(yùn)動(dòng)神經(jīng)通路中的相應(yīng)神經(jīng)元進(jìn)行良好環(huán)境刺激,有利于提高神經(jīng)細(xì)胞的興奮性,能夠促進(jìn)神經(jīng)細(xì)胞軸突和樹(shù)突的形成及神經(jīng)纖維的髓鞘化,不僅能夠使受損的結(jié)構(gòu)出現(xiàn)功能性適應(yīng),同時(shí)也能夠促進(jìn)神經(jīng)細(xì)胞的修復(fù)、橋接和重組,減少神經(jīng)組織損傷引起的智能發(fā)育障礙,加快受損的神經(jīng)功能恢復(fù),進(jìn)而呈現(xiàn)出明顯的追趕趨勢(shì)[13-16]。

早期康復(fù)訓(xùn)練對(duì)腦損傷綜合征早產(chǎn)兒的干預(yù)主要通過(guò)視覺(jué)、聽(tīng)覺(jué)、觸覺(jué)、平衡感、被動(dòng)操與康復(fù)訓(xùn)練等方法進(jìn)行刺激,通過(guò)外周感受器進(jìn)入中樞神經(jīng)系統(tǒng),可刺激大腦皮層發(fā)育產(chǎn)生反應(yīng),促進(jìn)大腦某些區(qū)域代償性神經(jīng)細(xì)胞再生,有利于促進(jìn)腦部受損神經(jīng)功能的恢復(fù),促進(jìn)其大腦、小腦及眼睛深部感覺(jué)發(fā)育和感覺(jué)系統(tǒng)平衡,盡可能恢復(fù)受損的神經(jīng)功能,促進(jìn)患兒體格和智力的發(fā)育,增強(qiáng)患兒的社會(huì)及日常生活能力[17-19]。與此同時(shí),神經(jīng)系統(tǒng)的較好發(fā)育也可有效減少或消除患兒運(yùn)動(dòng)上的功能障礙,而運(yùn)動(dòng)功能的恢復(fù)也可進(jìn)一步促進(jìn)肢體及智力的發(fā)育及協(xié)調(diào)能力,進(jìn)一步促進(jìn)受損神經(jīng)功能的恢復(fù),形成一良性循環(huán),挖掘患兒內(nèi)在的潛能,改善患兒生長(zhǎng)發(fā)育及智力水平[20-25]。本研究發(fā)現(xiàn)干預(yù)12個(gè)月和24個(gè)月后,干預(yù)組早產(chǎn)兒的身高、體質(zhì)量和智力發(fā)育商水平[(76.12±5.87)cm、(85.13±6.03)cm、(9.62±0.95)kg、(11.23±1.03)kg、(92.17±6.17)分、(94.25±7.13)分]均明顯高于對(duì)照組早產(chǎn)兒[(73.12±4.24)cm、(82.72±7.68)cm、(8.58±0.89)kg、(9.98±0.92)kg、(88.12±5.14)分、(90.45±6.02)分]。提示早期康復(fù)訓(xùn)練用于早產(chǎn)兒腦損傷綜合征患者可修復(fù)神經(jīng)細(xì)胞損傷,使其腦功能得到有效恢復(fù),促進(jìn)早產(chǎn)兒的生長(zhǎng)發(fā)育,使患兒機(jī)體生長(zhǎng)、智能發(fā)育等各方面得到明顯改善。同時(shí)研究發(fā)現(xiàn)干預(yù)24個(gè)月后,干預(yù)組早產(chǎn)兒殘留的神經(jīng)系統(tǒng)后遺癥的發(fā)生率(7.46%)明顯低于對(duì)照組(19.57%)。提示早期康復(fù)訓(xùn)練用于早產(chǎn)兒腦損傷綜合征患者,降低神經(jīng)系統(tǒng)后遺癥的發(fā)生率。

總之,早產(chǎn)兒腦損傷綜合征患者予以早期康復(fù)訓(xùn)練可修復(fù)受損的神經(jīng)細(xì)胞,促進(jìn)其生長(zhǎng)發(fā)育,降低神經(jīng)系統(tǒng)后遺癥的發(fā)病率,有利于改善患兒的預(yù)后。
