丹參酮ⅡA對血瘀型大鼠急性心梗后心功能的影響

[摘要] 目的 探討丹參酮ⅡA對急性心梗大鼠再灌注損傷的保護作用。 方法 選取30只SD大鼠,建立血瘀模型,隨機分為假手術(shù)組(Sham組)、AMI對照組(Control組)及丹參酮ⅡA干預(yù)組(TSN組)。Sham組只開胸穿線不結(jié)扎,Control組與TSN組建立心梗再灌注模型。造模前、再灌注4周后觀察各組的超聲心動圖,再灌注4周后比較各組的血流動力學(xué)結(jié)果。 結(jié)果 超聲心動圖結(jié)果顯示Control組的左室收縮末內(nèi)徑(LVIDs)和左室舒張末內(nèi)徑(LVIDd)較Sham組擴大(P<0.05),左室射血分數(shù)及左室短軸縮短率較假手術(shù)組降低(P<0.05);TSN組與Control組相比,LVIDs及LVIDd有改善(P<0.05),且LVEF%及FS%得到提高(P<0.05)。血流動力學(xué)結(jié)果顯示,再灌注4周后,Control組與假手術(shù)組比較,可見HR加快、LVSP下降、LVEDP升高(P<0.05);與Control組比較,TSN組能升高LVSP,降低LVEDP,差異有統(tǒng)計學(xué)意義(P<0.05),但HR則無差異(P>0.05)。 結(jié)論 丹參酮ⅡA可改善血瘀型大鼠急性心梗后的心功能。

Impacts of tanshinone ⅡA on cardiac function after acute myocardial infarction in blood stasis rats

[Abstract] Objective To investigate the salvage efficacy of tanshinone ⅡA on reperfusion injury in rats with acute myocardial infarction. Methods Thirty SD rats were randomly divided into sham operation group (the sham group), AMI control group (the control group) and tanshinone ⅡA intervention group (the TSN group) to establish blood stasis model. In the sham group, ?only thoracotomy was performed without ligation. In the control group and the TSN group, ?myocardial infarction reperfusion model was established. Echocardiography of each group was observed before modeling and four weeks after reperfusion, and hemodynamic results of each group were compared four weeks after reperfusion. Results Echocardiographic results showed that the left ventricular internal dimension in systole(LVIDs) and left ventricular internal dimension-diastole (LVIDd) in the control group were larger than those in the sham group(P<0.05), ?and the left ventricular ejection fraction and left ventricular short-axis shortening rate were lower than those in the sham group(P<0.05). Compared with the control group, LVIDs and LVIDd in the TSN group were improved(P<0.05), and LVEF% and FS% were improved(P<0.05). Hemodynamic results showed that HR increased, ?LVSP decreased and LVEDP increased in the control group compared with the sham operation group after four weeks of reperfusion(P<0.05). Compared with the control group, ?the TSN group increased LVSP and decreased LVEDP with statistically significant differences(P<0.05), ?but HR had no statistically significant difference(P>0.05). Conclusion Tanshinone ⅡA improves cardiac function after acute myocardial infarction in blood stasis rats.

2.3 三組血流動力學(xué)檢測

再灌注4周后,AMI對照組(Control組)與假手術(shù)組(Sham組)比較,可見心率(HR)加快、左室收縮壓(LVSP)下降、左室舒張末壓(LVEDP)升高,差異有統(tǒng)計學(xué)意義(P<0.05)。與AMI對照組(Control組)比較,丹參酮ⅡA干預(yù)組(TSN組)能升高LVSP,降低LVEDP,具有統(tǒng)計學(xué)意義(P<0.05),但HR則無差異(P>0.05)。見表3。

2.4 Control組與TSN組心肌無復(fù)流面積和心肌梗死面積比較

再灌注4周后,Control組與TSN組相比缺血面積比相似(P>0.05),表明各組間基線一致,其梗死面積及無復(fù)流面積存在可比性。與Control組相比,TSN組的梗死面積和無復(fù)流面積縮小,有統(tǒng)計學(xué)差異(P<0.05)。見表4。

3 討論

隨著血運重建技術(shù)的發(fā)展,急性心肌梗死的死亡率得到了下降,但缺血再灌注損傷然仍是臨床的重要課題[9]?,F(xiàn)代研究顯示缺血再灌注損傷主要機制與氧化應(yīng)激、鈣超載、炎癥因子及線粒體膜通透性等存在相關(guān)性[10-13],但臨床如何有效改善缺血再灌注損傷仍待深入探索,尤其是如何維護缺血損傷后的心功能更是臨床重點。

我國傳統(tǒng)中藥具有多靶點、多作用機制的特點,在改善缺血再灌注損傷中發(fā)揮著重要的作用。祖國醫(yī)藥不僅能改善心肌缺血癥狀,還通過各種機制發(fā)揮抗缺血再灌注作用。研究顯示,中醫(yī)藥可通過氧化應(yīng)激相關(guān)機制改善心肌缺血再灌注損傷[14];同時也證明中醫(yī)藥可提升SOD的活力,降低MDA的含量,減輕心肌細胞損傷[15];亦有研究顯示可改善心肌細胞中的炎癥因子,并通過增加蛋白水平發(fā)揮抗損傷作用[16];并有研究認為中醫(yī)藥可改善大鼠心肌梗死,且機制與C反應(yīng)蛋白與髓過氧化物酶相關(guān)[17]。

在改善急性心梗后心功能方面,祖國醫(yī)學(xué)認為心力衰竭的發(fā)生其病機與血瘀密不可分,故往往采用活血化瘀為治法改善心功能。

丹參作為活血化瘀的代表藥物,味苦,性微寒,歸心、肝經(jīng),具有活血通絡(luò)、祛瘀止痛、涼血消癰、清除心煩等功效。李時珍的《本草綱目》記載“丹參活血,通心包絡(luò)”。丹參的藥物主要有效成分分為水溶性成分(以丹參素和丹酚酸為代表)和脂溶性成分(以丹參酮為代表)。丹參酮ⅡA是丹參酮最具活性的化學(xué)結(jié)構(gòu),其具有更優(yōu)的活血化瘀作用,還具有改善微循環(huán)、擴張冠狀動脈、抑制血栓素生成、減少血小板黏附和聚集等作用,被廣泛應(yīng)用于心腦血管等領(lǐng)域。有研究表明,丹參酮ⅡA具有抗動脈粥樣硬化作用,能減少心肌缺血和心肌缺血再灌注損傷[18-19];丹參酮ⅡA通過抑制ERK1/2MAPK信號抑制AGEs誘導(dǎo)的血管平滑肌細胞增殖和遷移[20]。

本研究組以丹參酮ⅡA為研究對象,項目組既往研究已證實其能縮小梗死面積,改善再灌注損傷[7-8]。本研究進一步探討丹參酮ⅡA對心梗后心功能的保護作用。研究結(jié)果提示:大鼠心梗再灌注4周后,心超顯示丹參酮ⅡA干預(yù)組能有效降低LVIDs及LVIDd,并提高LVEF%及FS%,同時,血流動力學(xué)結(jié)果亦顯示,丹參酮ⅡA升高LVSP,降低LVEDP。兩者均提示丹參酮ⅡA對心梗后的心功能有保護作用,對比心肌無復(fù)流面積和心肌梗死面積,提示丹參酮ⅡA能改善心功能,且改善心肌缺血和再灌注損傷與心功能的影響呈正相關(guān)。

本研究存在一定的局限性:(1)本研究樣本量較小,丹參酮ⅡA對心功能的作用有待更多的研究進一步證實。(2)研究未涉及丹參酮ⅡA的作用機制,研究組下階段擬擴大樣本量,進行深化機制研究。
