[摘要] 目的 探討胃癌患者血清白細(xì)胞介素(IL)-17、同型半胱氨酸(Hcy)水平與病理特征的關(guān)系及臨床診斷價(jià)值。 方法 選取河北省唐山市人民醫(yī)院2015年3月—2017年3月收治的胃癌患者80例為胃癌組,另選擇胃良性病變患者80例為良性病變組,同期健康體檢志愿者80名為正常對照組。比較各組血清IL-17、Hcy水平,分析胃癌患者血清IL-17、Hcy與病理特征的關(guān)系,繪制ROC曲線評(píng)價(jià)IL-17、Hcy聯(lián)合檢測對胃癌預(yù)測價(jià)值。結(jié)果 胃癌組血清IL-17、Hcy水平高于良性病變組及對照組,良性病變組血清IL-17、Hcy水平高于正常對照組(P < 0.05);胃癌患者血清IL-17、Hcy水平與浸潤深度、淋巴結(jié)轉(zhuǎn)移、TNM分期有關(guān)(P < 0.05);IL-17與Hcy聯(lián)合檢測的曲線下面積、敏感度、特異度及約登指數(shù)均高于IL-17、Hcy 單獨(dú)檢測結(jié)果(P < 0.05)。 結(jié)論 IL-17、Hcy在胃癌患者血清中具有高表達(dá),均與胃癌浸潤深度、淋巴結(jié)轉(zhuǎn)移及TNM分期有關(guān),IL-17、Hcy聯(lián)合檢測可提高早期胃癌的診斷效果。

[Abstract] Objective To explore the relationship between serum interleukin (IL)-17, homocysteine (Hcy) levels and pathological features in patients with gastric cancer and its clinical diagnostic value. Methods A total of 80 patients with gastric cancer admitted to Tangshan People′s Hospital from March 2015 to March 2017 were selected as gastric cancer group, another 80 patients with benign gastric lesions were selected as the benign lesion group, and 60 volunteers with health examination during the same period were selected as the normal control group. The serum IL-17 and Hcy levels in each group were compared, and the relationship between serum IL-17 and Hcy and pathological features in gastric cancer patients was analyzed. ROC curve was drawn to evaluate the predictive value of combined IL-17 and Hcy detection for gastric cancer. Results The serum IL-17 and Hcy levels in gastric cancer group were higher than those in the benign lesion group and the normal control group, and serum IL-17 and Hcy levels in the benign lesion group were higher than those in the normal control group (P < 0.05). The serum IL-17 and Hcy levels in the gastric cancer patients were related to infiltration depth, lymph node metastasis and TNM stage (P < 0.05). The area under the curve, sensitivity, specificity and Youden index of IL-17 combined with Hcy detection were all higher than those of IL-17 detection and Hcy detection separately (P < 0.05). Conclusion IL-17 and Hcy are highly expressed in the serum of patients with gastric cancer, which are related to the depth of invasion, lymph node metastasis and TNM stage of gastric cancer. Combined detection of IL-17 and Hcy can improve the diagnosis effect of early gastric cancer.
