復(fù)雜型腹主動脈瘤血管腔內(nèi)介入治療效果分析及評價

目的 通過對患者病情、術(shù)前相關(guān)檢查、手術(shù)方法、術(shù)后治療措施及隨訪這幾方面進(jìn)行回顧性研究,討論分析應(yīng)用血管腔內(nèi)介入技術(shù)治療復(fù)雜型腹主動脈瘤的治療效果。方法 將2011年1月至2015年6月就診于哈爾濱醫(yī)科大學(xué)附屬第二醫(yī)院血管外科并行手術(shù)治療的156例患者的病情、術(shù)前相關(guān)檢查、手術(shù)方法、術(shù)后治療措施等資料整理收集,共將其分為2組:復(fù)雜型腹主動脈瘤患者組(A組),共63例,普通型腹主動脈瘤患者組(B組),共93例,將這些患者的臨床資料,整理總結(jié),并進(jìn)行分析。結(jié)果 2組手術(shù)成功率均為100%。圍手術(shù)期及隨訪期間,復(fù)雜型腹主動脈瘤組出現(xiàn)1例死亡,圍術(shù)期內(nèi)出現(xiàn)內(nèi)漏11例,經(jīng)治療內(nèi)漏均消失;隨訪期發(fā)生4例內(nèi)漏,多數(shù)經(jīng)保守觀察后消失,僅1例需治療。普通型腹主動脈瘤組未出現(xiàn)死亡病例,圍術(shù)期內(nèi)出現(xiàn)內(nèi)漏14例,經(jīng)治療內(nèi)漏均消失;隨訪期內(nèi)出現(xiàn)內(nèi)漏6例,經(jīng)保守治療觀察期內(nèi)漏消失。計算機(jī)斷層掃描血管造影(CTA)檢查證實在術(shù)后及隨訪期間2組患者無明顯支架移位發(fā)生。結(jié)論 復(fù)雜型腹主動脈瘤組與普通型腹主動脈瘤組,在住院時間、手術(shù)時間、遠(yuǎn)期存活率及并發(fā)癥的發(fā)生率2組無明顯差異。血管腔內(nèi)介入技術(shù)能夠有效的治療復(fù)雜型性腹主動脈瘤。

腹主動脈瘤(abdominal aortic aneurysm, AAA)多數(shù)治療并不能取得滿意的效果[1]。血管腔內(nèi)介入治療的出現(xiàn)[2]使AAA得以有效的治療,整體生存率明顯提高。AAA患者多為老年患者,對于常規(guī)手術(shù)不能耐受[3],既往存在高血壓、冠心病、心梗、腦梗等病史。腔內(nèi)修復(fù)術(shù)(endovascular aneurysm repair, EVAR)手術(shù)相較于傳統(tǒng)開放手術(shù)而言[4],其創(chuàng)傷小,恢復(fù)快,患者可早期下地活動,并可減少或無需術(shù)中輸血,同時術(shù)后監(jiān)護(hù)時間縮短,患者整體住院時間縮短,EVAR能減少手術(shù)對于患者的創(chuàng)傷。隨訪發(fā)現(xiàn)盡管開放手術(shù)的方式在腎動脈水平以上鉗夾主動脈出現(xiàn)的風(fēng)險較低[6],但其術(shù)后產(chǎn)生的并發(fā)癥與死亡率較高,面對這樣的患者時,許多術(shù)者拒絕施行開放性手術(shù)[7]。應(yīng)用EVAR治療復(fù)雜型腹主動脈瘤逐漸成為最佳的治療方法。但EVAR能否使復(fù)雜型腹主動脈瘤患者以較小的代價獲得較大的收益,臨床上雖然進(jìn)行了大量的隨訪研究,但對其有效性和可靠性仍需長期觀察[8]。近幾年,新型開窗—分支型支架的出現(xiàn)使AAA的治療有了更好的發(fā)展[9],目前多數(shù)仍處于臨床試驗研究階段,但在不久的將來,隨著不斷改進(jìn)和發(fā)展,越來越多的患者將能夠接受開窗—分支支架的治療。新型支架的發(fā)展必將為AAA的治療帶來更好的前景[10]。
1.1 研究對象
對2011年1月至2015年6月就診于哈爾濱醫(yī)科大學(xué)附屬第二醫(yī)院血管外科并行手術(shù)治療的156例患者的病情、術(shù)前相關(guān)檢查、手術(shù)方法、術(shù)后治療措施等資料進(jìn)行整理收集。將156例患者共分2組,A組為復(fù)雜型腹主動脈瘤組,63例;B組為普通型腹主動脈瘤組,93例。所有患者均經(jīng)主動脈計算機(jī)斷層掃描血管造影(CTA)、數(shù)字減影血管造影(DSA)等明確診斷。
1.2排納標(biāo)準(zhǔn)
入選標(biāo)準(zhǔn):術(shù)前所有患者均行主動脈CTA檢查,主要包括觀察動脈瘤形態(tài),是否呈明顯偏心性生長,近端錨定區(qū)長度、成角畸形等;瘤體長度及直徑;腹主動脈分叉部直徑;腎動脈開口至腹主動脈分叉處長度、腹主動脈分叉處至雙側(cè)髂內(nèi)動脈開口處長度等。經(jīng)測量及3維影像觀察,將患者分為復(fù)雜型腹主動脈瘤(A)組和普通型腹主動脈瘤(B)組。A組診斷標(biāo)準(zhǔn)主要包括:瘤頸成角扭曲;近端錨定區(qū)處瘤頸長度<15 mm,近端瘤頸直徑>30 mm;瘤頸成角>45°;瘤頸處鈣化面積≥25%或主動脈壁附壁血栓面積≥50%;腎動脈旁及腎上腹主動脈瘤;累積髂血管的腹主動脈瘤;入路狹窄、腎動脈狹窄;動脈瘤破裂及假性動脈瘤。符合以上標(biāo)準(zhǔn)的患者為A組,其余患者收入B組。
排除標(biāo)準(zhǔn):感染性腹主動脈瘤、多發(fā)性大動脈炎、自身免疫性疾病等。
1.3 手術(shù)過程
除急診手術(shù)外,一般術(shù)前均給予內(nèi)科治療,控制血壓、心率等至平穩(wěn)狀態(tài),待患者狀態(tài)穩(wěn)定。術(shù)前精確測量多部位主動脈直徑,觀察動脈瘤瘤頸及瘤體形態(tài),備選腔內(nèi)及人工血管移植物,決定施行EVAR或雜交手術(shù)。全麻下氣管插管,確切術(shù)野下逐層切開顯露雙側(cè)股總動脈近、遠(yuǎn)端套帶備用或預(yù)置雙縫合器備用,靜脈推注肝素,實現(xiàn)全身肝素。術(shù)前以CTA軸位圖像測得的直徑做為主動脈主體覆膜支架選擇的標(biāo)準(zhǔn),術(shù)中結(jié)合造影結(jié)果,再次對支架的選擇進(jìn)行評估。分別釋放主體與髂肢支架。支架釋放完成后,需再次進(jìn)行造影觀察支架形態(tài)與位置是否有內(nèi)漏存在、動脈瘤是否隔絕完全。將導(dǎo)管連同導(dǎo)絲一起撤出動脈,縫合動脈壁穿刺處,逐層縫合皮膚及皮下組織;預(yù)置縫合器患者行縫合器縫合并剪斷,加壓包扎創(chuàng)口,轉(zhuǎn)入ICU監(jiān)護(hù)。
1.4 術(shù)后治療與隨訪
術(shù)后患者均轉(zhuǎn)入ICU病房監(jiān)護(hù),常規(guī)為24 h。對于高血壓患者,早期靜脈輸注藥物降壓治療,待患者血壓平穩(wěn)后,逐步改為口服降壓藥控制,并觀察其效果,叮囑患者長期口服降壓藥物,維持血壓平穩(wěn)。
本次入組患者術(shù)后隨訪期為0~24個月。以電話的形式通知其隨訪治療,隨訪結(jié)果均以門診復(fù)診的主動脈CTA為主。
1.5 資料采集
1.5.1一般信息采集
(1)患者基本情況;
(2)患者入院情況如首發(fā)癥狀,伴隨癥狀如下肢血運,腹腔臟器灌注,代謝情況,住院天數(shù)等;
(3)手術(shù)操作過程如具體手術(shù)及麻醉方式、時間,血管腔內(nèi)移植物型號規(guī)格及術(shù)中特殊情況記錄等;
(4)圍手術(shù)期患者實驗室(如肝、腎功指標(biāo))及影像學(xué)(如CTA)檢查,生命體征變化情況、術(shù)后并發(fā)癥等;
(5)隨訪內(nèi)容包括并發(fā)癥的發(fā)生與治療,原發(fā)疾病是否復(fù)發(fā)等。
1.5.2 影像學(xué)信息收集
徑線測量:(1)近端瘤頸內(nèi)徑;(2)動脈瘤最大直徑;(3)瘤體遠(yuǎn)端主動脈直徑;(4)分叉部位血管直徑; (5)髂總動脈內(nèi)徑;(6)近端瘤頸的長度; (7)瘤體上緣至下緣的長度。

1.6 統(tǒng)計學(xué)分析
收集整理所有患者的住院病歷及隨訪資料,用均數(shù)±標(biāo)準(zhǔn)差表示正態(tài)分布的計量資料,采用t檢驗;樣本比較率采用Fisher確切概率法。應(yīng)用SPSS 17.0國際標(biāo)準(zhǔn)統(tǒng)計學(xué)編程軟件進(jìn)行統(tǒng)計分析。P>0.05表明無明顯統(tǒng)計學(xué)意義,P<0.05表明差異有明顯統(tǒng)計學(xué)意義。
患者在疼痛的癥狀、動脈瘤長度及直徑方面有明顯差異(P<0.05),在既往史方面無明顯統(tǒng)計學(xué)意義。但復(fù)雜型腹主動脈瘤組患者年齡明顯高于普通型動脈瘤組,且平均手術(shù)時間、平均住院時間較普通型動脈瘤組長(表1)。
復(fù)雜型腹主動脈瘤(A組)組與普通型腹主動脈瘤組(B組)在死亡率、內(nèi)漏發(fā)生率、血管損傷比率方面無明顯差異(表2)。

3.1 數(shù)據(jù)分析討論
復(fù)雜型腹主動脈瘤患者組與普通型腹主動脈瘤患者組2者進(jìn)行比較,雖然在年齡、性別、住院時間、基礎(chǔ)疾病等方面未出現(xiàn)明顯差異。但可以看到的是相較于普通型動脈瘤組而言,復(fù)雜型動脈瘤組平均年齡高,解剖條件較復(fù)雜,在瘤體直徑及瘤體長度上有明顯差異,有腹部疼痛主訴的患者較多,均提示其治療的高危性及復(fù)雜性。從平均手術(shù)時間來看,復(fù)雜型腹主動脈瘤組除其中3例患者施行雜交手術(shù)外,其余患者均行EVAR手術(shù),與普通型腹主動脈瘤組相比較,其平均手術(shù)時間更長。這是由于解剖條件的復(fù)雜性,包括入路動脈的狹窄、成角、閉塞;髂動脈夾層、動脈瘤形成;瘤體成角較大;部分存在較大的附壁血栓。研究結(jié)果顯示:復(fù)雜型腹主動脈瘤組及普通型腹主動脈瘤組均出現(xiàn)隨訪期內(nèi)漏,多數(shù)經(jīng)觀察隨訪自行封閉,僅1例需再次治療。2組患者均有支架內(nèi)阻塞出現(xiàn)。在總體生存率上,普通型組優(yōu)于復(fù)雜腹主動脈瘤組。
3.2內(nèi)漏治療討論
EVAR手術(shù)中最主要的并發(fā)癥為內(nèi)漏[11]。相關(guān)隨訪[12]表明隨訪期內(nèi)有漏并接受再次干預(yù)的患者,動脈瘤從第2年開始增長。無內(nèi)漏的患者,動脈瘤尺寸持續(xù)下降。內(nèi)漏的發(fā)生與治療是評價手術(shù)與治療效果的一項重要標(biāo)準(zhǔn)。過去,內(nèi)漏的存在是限制EVAR應(yīng)用于復(fù)雜型腹主動脈瘤的原因之一[13]。現(xiàn)在,隨著相關(guān)經(jīng)驗的積累及支架移植物的改進(jìn),使得內(nèi)漏的發(fā)生率降低,并得到很好的治療[14]。從表2和表3的數(shù)據(jù)可以看出,雖然復(fù)雜型腹主動脈瘤組較普通型腹主動脈瘤組內(nèi)漏發(fā)生率高,卻無明顯統(tǒng)計學(xué)意義。從相關(guān)經(jīng)驗來看,首先在支架的選擇上,oversize要適當(dāng)選擇[15],瘤頸不良者更應(yīng)該適當(dāng)放大,結(jié)構(gòu)不良性疾病應(yīng)該縮小。球囊擴(kuò)張治療、cuff支架及裸支架植入、反流血管彈簧圈栓塞等均可有效治療內(nèi)漏。
由于近年來隨著經(jīng)驗的積累,介入器材的不斷改進(jìn)、支架種類的豐富及設(shè)計的進(jìn)步、EVAR技術(shù)的不斷發(fā)展等使得復(fù)雜型腹主動脈瘤通過血管腔內(nèi)介入技術(shù)進(jìn)行治療的想法得以實現(xiàn),且取得了滿意的治療效果。在多項指標(biāo)的比較上可見,復(fù)雜型腹主動脈瘤組在治療效果、并發(fā)癥、生存率等方面與普通型動脈瘤組無明顯差異。

Analysis and evaluation of endovascular interventional treatment of complex abdominal aortic aneurysm

Objective Retrospectively analyse and evaluate the therapeutic effect of endovascular interventional technique in the treatment of complicated abdominal aortic aneurysm according to the patients' condition, preoperative examination, surgical methods, postoperative treatment measures and follow-up data. Methods Collecting the data which including the condition, preoperative examination, surgical methods, and postoperative treatment measures of patients who are treated in the department of vascular surgery, Second Affiliated Hospital of Harbin Medical Universityfrom 2011 January to 2015 June. Divided the patients into two groups: Complex abdominal aortic aneurysm patients group (A group), a total of 63 cases; abdominal aortic aneurysm patients group (B group), a total of 93 cases. Collate and summarize the clinical data of these patients. Results The success rates of the two groups were 100%. During the perioperation period and follow-up period, complicated abdominal aortic aneurysm group appeared 1 cases died. 11 cases of endoleaks was found in perioperative period. After treatment the endoleaks disappeared. 4 cases of endoleaks were found during follow-up period, only 1 case need treatment. The common type of abdominal aortic aneurysm group has no deaths occurred in the perioperative period and 14 cases of endoleaks occurred. Endoleaks disappeared after treatment. 6 cases appeared during the follow-up period. All of the endoleaks disappeared during the observation period. CTA examination showed that there were no significant stent shift occurred in the two groups after the operation and follow-up period. Conclusions There were no significant differences in the length of stay and operation time between the complex abdominal aortic aneurysm group and the common type of abdominal aortic aneurysm. The incidence rate of long-term survival rate and complication of complicated abdominal aortic aneurysm group and common type of abdominal aortic aneurysm group had no significant difference. Endovascular interventional techniques can be effective in the treatment of complex abdominal aortic aneurysm.
