【摘要】 目的：對(duì)比青光眼合并白內(nèi)障的不同聯(lián)合手術(shù)的治療效果。方法：于筆者所在醫(yī)院2017年10月-2018年10月收治的青光眼合并白內(nèi)障患者中隨機(jī)選取84例作為樣本。采用隨機(jī)數(shù)字表法將其分為兩組，每組42例。對(duì)照組行白內(nèi)障囊外摘除人工晶體置入+青光眼小梁切除術(shù)治療，觀察組行白內(nèi)障超聲乳化人工晶狀體置入+小梁切除術(shù)治療。比較兩組臨床療效、眼壓、視力及并發(fā)癥發(fā)生率。結(jié)果：觀察組總有效率為92.86%、術(shù)后眼壓（10.59±1.13）mm Hg、視力（0.76±0.10）、并發(fā)癥發(fā)生率為2.38%。對(duì)照組總有效率73.81%、術(shù)后眼壓（32.36±1.52）mm Hg、視力（0.40±0.12）、并發(fā)癥發(fā)生率為19.05%。兩組比較差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論：行白內(nèi)障超聲乳化人工晶狀體置入+小梁切除術(shù)治療青光眼合并白內(nèi)障，效果顯著，并發(fā)癥少，能夠有效降低眼壓，改善視力，從而提升患者的生活質(zhì)量。

Comparative Analysis of Different Combined Surgery Methods for Glaucoma Complicated with Cataract/LIN Yuni. //Chinese and Foreign Medical Research， 2020， 18（10）： -128

[Abstract] Objective： To compare the effect of different combined surgery for glaucoma complicated with cataract. Method： Among the glaucoma complicated with cataract patients admitted in our hospital from October 2017 to October 2018， 84 cases were randomly selected as the samples. They were divided into two groups by random number table method， with 42 cases in each group. The control group was treated with extracapsular cataract extraction and intraocular lens implantation and trabeculectomy for glaucoma， and the observation group was treated with phacoemulsification and intraocular lens implantation and trabeculectomy. The clinical efficacy， intraocular pressure， visual acuity and incidence of complications were compared between the two groups. Result： The total effective rate in the observation group was 92.86%， postoperative intraocular pressure was （10.59±1.13） mm Hg， visual acuity was （0.76±0.10）， and the incidence of complications was 2.38%. The total effective rate in the control group was 73.81%， postoperative intraocular pressure was （32.36±1.52） mm Hg， visual acuity was （0.40±0.12）， and the incidence of complications was 19.05%. The differences between the two groups were statistically significant （P<0.05）. Conclusion： Phacoemulsification and intraocular lens implantation and trabeculectomy in the treatment of glaucoma complicated with cataract has a significant effect and few complications， which can effectively reduce intraocular pressure， improve visual acuity and improve the quality of life of patients.

青光眼及白內(nèi)障均為臨床常見的眼科疾病，當(dāng)白內(nèi)障處于發(fā)展期、成熟期時(shí)，患者罹患青光眼的風(fēng)險(xiǎn)一般較高[1]。導(dǎo)致上述現(xiàn)象出現(xiàn)的原因，與晶體膨脹向前推擠眼內(nèi)房水外流，以及混濁晶體成分溢出到眼內(nèi)誘發(fā)炎性反應(yīng)有關(guān)[2]。因此，部分眼科患者青光眼可與白內(nèi)障一同發(fā)病，導(dǎo)致治療難度增加，致使視力下降，影響患者的生活質(zhì)量[3]。青光眼合并白內(nèi)障的治療術(shù)式，包括白內(nèi)障囊外摘除人工晶體置入+青光眼小梁切除術(shù)、白內(nèi)障超聲乳化人工晶狀體置入+小梁切除術(shù)兩種[4]。有學(xué)者在研究中指出，根據(jù)手術(shù)方式的不同，臨床療效也不同[5]。本文于筆者所在醫(yī)院2017年10月-2018年10月收治的青光眼合并白內(nèi)障患者中，隨機(jī)選取84例作為樣本，對(duì)比不同聯(lián)合手術(shù)方式的臨床療效，現(xiàn)報(bào)道如下。

綜上所述，行白內(nèi)障超聲乳化人工晶狀體置入+小梁切除術(shù)治療青光眼合并白內(nèi)障，效果顯著，并發(fā)癥少，能夠有效降低眼壓，改善視力，從而提升患者的生活質(zhì)量。
