【摘要】 目的:對生長分化因子-15水平(growth differentiation factor-15,GDF-15)與急性冠脈綜合征(acute coronary syndrome,ACS)患者心功能分級的臨床意義進行研究。方法:58例ACS患者按紐約心臟病協(xié)會(NYHA)的心功能分級方法分為NYHA Ⅰ級組(21例)、NYHA Ⅱ級組(22例)、NYHA Ⅲ級組(10例)、NYHA Ⅳ級組(5例)四個亞組。25例行冠脈造影排除冠心病的NYHA Ⅰ級患者為對照組。檢測各組B型利鈉肽(B-type natnuretic peptide,BNP)等生化指標,運用ELISA法測定GDF-15水平。結(jié)果:與對照組相比,ACS患者各亞組的GDF-15、BNP水平顯著高(P<0.01),且各組GDF-15、BNP水平隨著心力衰竭嚴重程度的升高而上升(P<0.05),GDF-15、BNP水平均與心衰程度呈正相關(guān)(r=0.681、0.653,P<0.001)。且GDF-15、BNP兩者間亦呈現(xiàn)正相關(guān)性(r=0.726,P<0.001)。結(jié)論:ACS患者的GDF-15水平升高顯著,與BNP呈正相關(guān),可對ACS患者的心功能進行較有價值的危險分層,利于判定ACS患者心力衰竭程度。

【Abstract】 Objective:To approach the clinical significance between the levels of growth differentiation factor-15(GDF-15) and the heart function classification in acute coronary syndrome(ACS).Method:58 ACS patients were divided into four sub-groups such as Ⅰ degree(21 cases),Ⅱ degree(22 cases),Ⅲ degree(10 cases),Ⅳ degree(5 cases) according to the New York heart association(NYHA).25 cases with Ⅰ degree of NYHA excluded coronary artery disease by coronary arteriography were treated as the control group.Biochemical indicators such as B-type natnuretic peptide(BNP) were tested in all groups.The levels of GDF-15 were determined by antibody-sandwich enzyme–linked immunosorbent assay(ELISA).Result:Compared with the control group,the analysis showed that the levels of GDF-15,BNP in ACS sub-groups were significantly increased(P<0.01),and the levels of GDF-15,BNP expression increased with the degree of heart failure increased(P<0.05),also the levels of GDF-15,BNP in ACS patients had a positive correlation with the degree of heart failure(r=0.681,0.653,P<0.001).Then the GDF-15 was positively correlated with BNP(r=0.726,P<0.001).Conclusion:The levels of GDF-15 in ACS patients are markedly increased,also has a positive correlation with BNP.They have more valuable to evaluate the risk stratification of the heart function,also can be used to judge the severity of heart failure in ACS patients.

急性冠脈綜合征(ACS)是心血管內(nèi)科常見的疾病之一,起病危急,是威脅著人類健康的多因素疾病。而ACS患者在疾病的后期常合并心力衰竭,這就使得患者的心功能、日常生活受到較嚴重的影響。如若能尋求較為特異的心血管生化指標來指導(dǎo)ACS早期診斷、危險分層,則對ACS的治療、判定預(yù)后意義重大。研究發(fā)現(xiàn),在創(chuàng)傷、炎癥因子、缺血缺氧等刺激下,GDF-15的表達顯著增加[1-4]。GDF-15與心血管疾病相關(guān),其作為對抗心血管損傷的保護因子而發(fā)揮其保護作用[5-6]。可作為心血管功能、疾病的新興標記物[7]。本文按照紐約心臟病協(xié)會(NYHA)的心功能分級方法對ACS患者實行相關(guān)分類,分析GDF-15水平與ACS患者心功能分級的意義,判斷其是否可作為ACS患者心功能嚴重程度的標記物,評估ACS患者心力衰竭程度及其對ACS預(yù)后的價值。endprint
