脊髓背根入髓區(qū)切開治療癌性神經(jīng)病理性疼痛的遠(yuǎn)期療效*

目的:探討脊髓背根入髓區(qū)(dorsal root entry zone, DREZ)切開術(shù)治療癌性神經(jīng)病理性疼痛的遠(yuǎn)期療效及安全性。方法:收集癌性神經(jīng)病理性疼痛病人4例,其中3例為直腸癌侵犯骶神經(jīng)致會陰區(qū)神經(jīng)病理性疼痛,1例為左側(cè)乳腺癌侵犯臂叢神經(jīng)導(dǎo)致上肢神經(jīng)病理性疼痛。4例均接受患側(cè)損傷和疼痛節(jié)段脊髓背根入髓區(qū)切開術(shù)治療,顯微鏡下用雙極電凝切開病變節(jié)段脊髓背根的入髓區(qū)。隨訪2年以上,分別于術(shù)后2周、術(shù)后每半年采用視覺模擬評分 (visual analogue scale, VAS) 評估手術(shù)療效。結(jié)果:術(shù)后2周,2例疼痛消失,2例疼痛緩解> 75%。隨訪2年,療效滿意率逐漸下降,2例疼痛緩解>75%,1例疼痛緩解60%,1例術(shù)后6月死亡。并發(fā)癥發(fā)生情況:同側(cè)下肢深感覺障礙3例,痛覺過敏2例,在術(shù)后6月隨訪時(shí)均有不同程度恢復(fù)。 結(jié)論:脊髓背根入髓區(qū)切開術(shù)是一種治療癌性神經(jīng)病理性疼痛的安全、有效的措施。

75%的癌癥病人通常都伴有慢性疼痛[1]。癌痛的治療首先選擇世界衛(wèi)生組織推薦的“藥物三階梯”治療,但是仍有部分病人頑固性疼痛對藥物治療無效,可通過不同的介入或手術(shù)治療緩解疼痛。對于癌性神經(jīng)病理性疼痛 (tumor-associated neuropathic pain, TaNP),可采用脊髓背根入髓區(qū)切開術(shù) (dorsal root entry zonotomy, DREZotomy 治療,手術(shù)通過切開疼痛和損傷節(jié)段的脊髓背根入髓區(qū) (dorsal root entry zone, DREZ)達(dá)到緩解疼痛的目的。2013年1月至2013年12月,福建醫(yī)科大學(xué)附屬第一醫(yī)院和深圳市第二人民醫(yī)院共收治TaNP病人4例,現(xiàn)回顧性分析其臨床資料,報(bào)道如下。
4例TaNP病人中,男性2例,女性2例,年齡分別為62、62、55和49歲,病程分別為8月、1年、3月和2年;直腸癌3例,乳腺癌1例;患肢完全癱瘓1例,不完全癱瘓3例。疼痛部位、性質(zhì)及術(shù)前用藥情況,見表1。入院前,除1例直腸癌病人余3例均接受過正規(guī)的放化療。4例均行頸椎和腰骶椎MRI檢查,均見相應(yīng)脊髓節(jié)段腫瘤侵犯(見圖1)。4例均在術(shù)前采用視覺模擬評分(visual analogue scale,VAS)進(jìn)行客觀定量評估,顯示VAS為8.0±0.82分。
(1)手術(shù)方法
4例均接受DREZotomy,采用氣管插管吸入麻醉,俯臥位。常規(guī)碘酒、酒精消毒鋪巾,根據(jù)疼痛范圍分別選擇頸后正中或胸腰后正中入路,在選定切開的節(jié)段行全椎板切除,徹底止血。顯微鏡下縱行切開硬脊膜,顯露脊髓的后外側(cè)面。術(shù)中在高倍顯微鏡下,根據(jù)萎縮神經(jīng)根(損傷的神經(jīng)根表現(xiàn)為萎縮、顏色晦暗、無光澤)的范圍確定需要行DREZotomy切開的范圍(見圖2)。在脊神經(jīng)背根根支進(jìn)入脊髓后外側(cè)溝入口的腹外側(cè)縱向切開軟脊膜,用顯微剝離子沿DREZ區(qū)鈍性分離,直達(dá)后角,可通過其顏色在顯微鏡下變?yōu)樽鼗疑右员鎰e。用顯微雙極電凝低功率燒灼,擴(kuò)大毀損范圍,一般深約3 mm(見圖3)。脊髓后外側(cè)動(dòng)脈走行于后外側(cè)溝中,其直徑為0.1~0.5 mm,該動(dòng)脈由后根動(dòng)脈發(fā)出,應(yīng)從后外側(cè)溝游離出來并加以保護(hù)。
(2)療效評價(jià)
術(shù)后2周對病人進(jìn)行VAS評分,記錄病人鎮(zhèn)痛藥物的服用量以及出現(xiàn)的并發(fā)癥。隨訪時(shí)間2年,每6個(gè)月通過門診和電話進(jìn)行隨訪,再次對病人進(jìn)行VAS評估,并記錄鎮(zhèn)痛劑的服用量(見表2)。
術(shù)后疼痛的改善率100% “完全消失”;術(shù)后疼痛的改善率75%~99%為“明顯減輕”;術(shù)后疼痛的改善率50%~74%為“減輕”,而術(shù)后疼痛的改善率< 50%為“療效差”。其中術(shù)后疼痛的改善率≥50%為“療效滿意”。
術(shù)后2周,4例均為療效滿意,其中,2例原有疼痛節(jié)段疼痛完全消失,VAS為0分;2例疼痛明顯減輕,VAS為1~2分。4例鎮(zhèn)痛劑的服用量均維持在術(shù)前水平,并于術(shù)后1月開始緩慢減量。長期隨訪結(jié)果顯示,病人止痛療效有所下降,在隨訪2年時(shí),仍有2例病人術(shù)后疼痛改善率 > 75%,1例病人術(shù)后疼痛改善率 > 50%,見表2。術(shù)后早期2例疼痛完全消失,VAS 0分,術(shù)后3個(gè)月完全停用鎮(zhèn)痛劑;2例疼痛明顯減輕,VAS 1~2分,鎮(zhèn)痛劑服用量明顯下降。
4例淺感覺減退或缺失區(qū)域均向上或向下擴(kuò)大1~2個(gè)皮節(jié),與手術(shù)毀損的DREZ節(jié)段相對應(yīng)。其中,2例出現(xiàn)一過性痛覺過敏,位于手術(shù)毀損節(jié)段的上1~2個(gè)皮節(jié),均于術(shù)后1月左右消失;3例出現(xiàn)術(shù)后同側(cè)下肢深感覺障礙和淺感覺減退或消失,深感覺障礙表現(xiàn)為行走時(shí)如踩棉花感,2例于術(shù)后1月內(nèi)恢復(fù),1例于術(shù)后3月恢復(fù)。4例均未出現(xiàn)術(shù)前肢體無力加重,無切口感染、愈合不良及腦脊液漏等其他并發(fā)癥。

癌痛的原因有腫瘤直接侵犯、神經(jīng)浸潤以及腫瘤治療引起的相關(guān)損傷(如放化療損傷、胸部手術(shù)導(dǎo)致肋間神經(jīng)損傷等)[2]。癌痛根據(jù)不同的疼痛病理生理機(jī)制可分為:感受傷害性疼痛、神經(jīng)病理性疼痛和混合性疼痛。常見的癌性神經(jīng)病理性疼痛有乳腺癌侵犯臂叢神經(jīng)損傷所致的臂叢神經(jīng)損傷后疼痛,即Pancoast綜合征;肺癌術(shù)后的肋間神經(jīng)痛;直腸癌侵犯馬尾神經(jīng)所致的馬尾神經(jīng)損傷后疼痛。本組3例病人為直腸癌侵犯馬尾神經(jīng)所致,1例病人為乳腺癌侵犯臂叢神經(jīng)損傷所致,神經(jīng)損傷可為完全性,也可為不完全性損傷。

對于藥物治療無效的難治性的癌痛病人,作為癌痛治療的“第四階梯”神經(jīng)外科手術(shù)有著廣闊的空間[3]。神經(jīng)外科治療癌痛的手術(shù)方式有以下幾種:脊髓背根入髓區(qū)切開術(shù)(dREZotomy),脊髓后正中點(diǎn)狀切開術(shù)(punctate midline myelotomy, PMM)[4],鞘內(nèi)嗎啡泵置入術(shù)(intrathecal morphine pump, IMP)[5]和脊髓電刺激術(shù)(spinal cord stimulation, SCS)[6]。對于感受傷害性疼痛,IMP是一種有效的治療方法;對于下腹部或盆腔的癌性內(nèi)臟痛,PMM可以作為治療的一種考慮。而對于TaNP,SCS和DREZotomy手術(shù)都可以作為治療的選擇,但應(yīng)嚴(yán)格選擇手術(shù)適應(yīng)證。對于腫瘤侵犯神經(jīng)或放化療造成神經(jīng)的完全性損傷,損傷節(jié)段感覺缺失或運(yùn)動(dòng)功能完全喪失的病人來說,SCS治療往往是無效的,此時(shí)DREZotomy將更為適合。對于神經(jīng)不完全性損傷,保留部分感覺和運(yùn)動(dòng)功能的病人來說,從保護(hù)病人的神經(jīng)功能考慮,SCS將是更好的治療選擇。
DREZotomy治療TaNP國內(nèi)外學(xué)者已有相關(guān)的研究報(bào)道[7~9]。DREZotomy術(shù)中,對于DREZ的毀損范圍包括背根分支周圍部分小的傷害性纖維、背外側(cè)束興奮性的內(nèi)側(cè)部分以及后角最外層(Rexed的I~V層)。大多數(shù)學(xué)者認(rèn)為,DREZotomy的原理是永久性破壞了傷害性傳入通路的二級神經(jīng)元,從而使疼痛緩解。但是,一些學(xué)者發(fā)現(xiàn),神經(jīng)病理性疼痛除了是對傷害性刺激傳入的反應(yīng)外,還與中樞神經(jīng)系統(tǒng)內(nèi)神經(jīng)元自發(fā)放電有關(guān),在一些神經(jīng)病理性疼痛病人中,脊髓后角電生理活動(dòng)異常活躍[9]。DREZotomy可同時(shí)消除脊髓后角的異常電生理活動(dòng)并且緩解疼痛[10,11]。
本研究4例TaNP病人中,術(shù)后早期疼痛完全消失2例,4例療效均滿意;2年長期隨訪中,仍有2例疼痛緩解75%以上,與大多數(shù)文獻(xiàn)報(bào)道一致[11~13]。由此證明,成功的DREZotomy可使大部分病人達(dá)到良好止痛。對于疼痛的性質(zhì)對手術(shù)效果的影響,Sindou等的研究顯示,DREZ切開術(shù)對有陣發(fā)性疼痛的病人療效較好(92%),而對于只有持續(xù)性疼痛的病人療效較差(63%)[12]。本組4例均表現(xiàn)為持續(xù)性疼痛陣發(fā)性加劇,術(shù)后陣發(fā)性疼痛均完全消失。DREZ似乎對陣發(fā)性疼痛效果更好,但持續(xù)性疼痛的存在并不是手術(shù)的禁忌證,這一點(diǎn)在術(shù)前選擇適應(yīng)證時(shí)至關(guān)重要。
文獻(xiàn)報(bào)道,DREZotomy的手術(shù)并發(fā)癥主要有患肢淺感覺減退或缺失范圍擴(kuò)大、同側(cè)下肢的深感覺和淺感覺障礙以及肌力下降、新出現(xiàn)的痛覺過敏、腦脊液漏等[11,13,14]。本研究的4例病人也出現(xiàn)不同程度的上述并發(fā)癥。在大多數(shù)情況下,這種損傷是由于術(shù)后早期水腫波及所致,而未必是完全性地不可逆性損傷。從本研究長期隨訪結(jié)果看出,大多數(shù)出現(xiàn)這類并發(fā)癥的病人在隨訪過程中都有不同程度地恢復(fù)。術(shù)后早期給予甲基強(qiáng)的松龍沖擊治療可以有效緩解脊髓水腫。另外,術(shù)中對脊髓微小血管的損傷,可能也是導(dǎo)致長束損傷的原因之一,所以術(shù)中應(yīng)注意保護(hù)脊髓后外側(cè)動(dòng)脈。本研究中2例病人出現(xiàn)毀損節(jié)段上1~2個(gè)皮節(jié)的痛覺過敏,這可能與DREZotomy毀損后角后造成新的傳入阻滯性疼痛有關(guān)。此類病人在術(shù)后早期應(yīng)用非甾體抗炎藥后,痛覺過敏一般在術(shù)后1~3月內(nèi)通常會消失。
綜上所述,DREZotomy是一種安全、有效的治療TaNP的方法,成功的手術(shù)可以使大部分病人達(dá)到良好止痛。

LONG-TERM EFFICACY OF DORSAL ROOT ENTRY ZONOTOMY FOR TUMORASSOCIATED NEUROPATHIC PAIN*

Objective: To analyze the long-term ef fi cacy and safety of dorsal root entry zonotomy (DREZotomy)in patients with tumor-associated neuropathic pain. Methods: Four patients with tumor-associated neuropathic pain (3 with colorectal cancer, 1 with breast cancer) were treated by DREZotomy. Microsurgical lesion was made with bipolar forceps along the DREZ. The patients were assessed by visual analogue scale (VAS) at 2 weeks, 6 months, 12 months, 18 months and 24 months after the surgery. Results: The greatest pain relief was reported at 2 weeks after the DREZ procedure. Two patients had 100% pain relief and two patients had more than 75% pain relief. Over 2-year follow-up, the rate of satisfaction was slightly decreased. Two patients had more than 75% pain relief and one patient had 60% pain relief, with one patient died at the 6 months after surgery because of colorectal cancer. The major complications included dysfunction of deep sensory over the ipsilateral lower extremity (n= 3)and hyperalgesia (n= 2). These complications were usually improved within 6 months after surgery. Conclusion:Microsurgical lesions in the dorsal root entry zone could be used for pain relief in patients with tumor-associated neuropathic pain. It is an effective and safe measure.
