摘要:目的? 觀察臍灸療法治療緩解期潰瘍性結(jié)腸炎(UC)的臨床療效,及其對患者結(jié)腸黏膜內(nèi)BTNL2-HLA信號通路調(diào)控情況,探討其治療UC可能的作用機(jī)制。方法? 采用隨機(jī)數(shù)字表法將70例患者分為常規(guī)組和臍灸組各35例。常規(guī)組予柳氮磺吡啶腸溶片1.5~2 g/d,口服;臍灸組在常規(guī)組基礎(chǔ)上行臍灸療法,每周2次。2組均連續(xù)治療12周。觀察2組臨床療效,采用生物信息學(xué)逆向挖掘與BTNL2相關(guān)的調(diào)控分子及互作網(wǎng)絡(luò)以確定其潛在的調(diào)控通路,實時熒光定量PCR分析臍灸療法對緩解期UC患者結(jié)腸黏膜內(nèi)相關(guān)靶點的調(diào)控。結(jié)果? 臍灸組完全緩解率為82.86%(29/35),常規(guī)組為51.43%(18/35),臍灸組明顯優(yōu)于常規(guī)組(P<0.05)。2組均可調(diào)控BTNL2-HLA信號通路關(guān)鍵靶點分子的表達(dá),臍灸組對HLA-DQA1和HLA-DRA的調(diào)控效果更佳。2組對HLA-DMA、HLA-DQA2、HLA-DRB1無調(diào)控效果。結(jié)論? 臍灸療法對UC患者的治療效果較好,BTNL2-HLA信號通路是其主要的作用通路之一。

Abstract: Objective To investigate the effects of umbilical moxibustion on the treatment of ulcerative colitis (UC) in remission stage; To analyze the regulation of BTNL2-HLA signaling pathway in the colonic mucosa of UC patients during the treatment; To speculate the mechanism of umbilical moxibustion in the treatment of UC. Methods Totally 70 patients were divided into control group and umbilical moxibustion group according to random number table method, with 35 cases in each group. The control group was given sulfasalazine enteric-coated tablets 1.5 to 2 g/d orally. In the umbilical moxibustion group, umbilical moxibustion was performed on the basis of the control group, twice a week. Both groups were treated continuously for 12 weeks. The clinical efficacy of the two groups was compared. The bioinformatics was used to reversely mine BTNL2-related regulatory molecules and interaction networks, and their potential regulatory pathways were further determined. RT-qPCR was used to analyze the regulation of umbilical moxibustion on the related targets in the colonic mucosa of the above UC patients. Results The complete remission rate was 82.86% (29/35) in the umbilical moxibustion group and 51.43% (18/35) in the control group. The umbilical moxibustion group was significantly better than the control group (P<0.05). Bothgroups could regulate the expression of key target molecules in BTNL2-HLA signaling pathway, and the umbilical moxibustion group had better control effect on HLA-DQA1 and HLA-DRA. The two groups had no regulation effect on HLA-DMA, HLA-DQA2 and HLA-DRB1. Conclusion Umbilical moxibustion has a good therapeutic effect on UC patients. BTNL2-HLA signaling pathway is one of the main pathways.
