摘 要 目的：觀察滾針放血療法聯(lián)合龍膽泄肝湯化載方治療肝膽濕熱型帶狀皰疹的療效。方法：收集2018年1月—2019年6月收治的肝膽濕熱型帶狀皰疹患者72例，隨機(jī)分為治療組和對(duì)照組各36例。對(duì)照組口服阿昔洛韋片0.8 g，每天3次；噴昔洛韋乳膏外擦患處，每天4次；口服甲鈷胺0.5 mg，每天3次，療程10 d。治療組在對(duì)照組基礎(chǔ)上加用滾針放血每3 d 1次，共3次；早、晚各口服龍膽瀉肝湯化載方湯藥250 ml，共14 d。比較兩組治療后視覺模擬評(píng)分法（VAS）評(píng)分、治愈率及后遺神經(jīng)痛發(fā)生率。結(jié)果：治療組治愈率為77.8%（28/36），明顯優(yōu)于對(duì)照組的44.4%（16/36，P<0.05）。治療組的后遺神經(jīng)痛發(fā)生率為8.3%（3/36），明顯低于對(duì)照組的33.3%（12/36，P<0.05）。治療后治療組的VAS評(píng)分為（0.67±0.76）分，明顯優(yōu)于對(duì)照組的（3.56±1.75）分（P<0.05）。結(jié)論：滾針放血療法聯(lián)合龍膽泄肝湯化載方治療帶狀皰疹可以有效緩解疼痛，縮短病程，提高治愈率，降低帶狀皰疹后遺神經(jīng)痛的發(fā)生率。

Observation of the effect of rolling needle bloodletting therapy combined with traditional Chinese medicine in the treatment of herpes zoster

ABSTRACT Objective： To observe the effect of rolling needle bloodletting therapy combined with Longdan xiegan decoction huazaifang in the treatment of herpes zoster（HZ） of hepatobiliary damp heat type. Methods： From January 2018 to June 2019， 72 cases of herpes zoster of hepatobiliary damp heat type were collected and randomly divided into a treatment group and a control group with 36 cases in each group. The control group took acyclovir tablets 0.8 g orally 3 times a day； penciclovir cream was applied to the affected area 4 times a day； mecobalamin 0.5 mg was given orally three times a day for 10 days. On the basis of the same treatment of the control group， the treatment group was given bloodletting by rolling needle once every 3 days， 3 times in total； in the morning and evening， 250 mml of Longdan xiegan decoction huazaifang was taken orally for 14 days. After treatment， visual analogue scale（VAS）， cure rate and incidence of sequelae neuralgia were compared between the two groups. Results： The cure rate of the treatment group was 77.8%（28/36）， which was significantly better than that of the control group 44.4%（16/36， P<0.05）. The incidence of sequelae neuralgia of the treatment group was 8.3%（3/36）， which was significantly lower than that of the control group 33.3%（12/36， P<0.05）. After treatment， the VAS score of the treatment group was （0.67 ±0.76）， which was significantly better than that of the control group（3.56±1.75）（P<0.05）. Conclusion： Rolling needle bloodletting therapy combined with Longdan xiegan decoction huazaifang can effectively relieve pain， shorten the course of disease， improve the cure rate， and reduce the incidence of postherpetic neuralgia.

綜上所述，滾針放血療法結(jié)合龍膽瀉肝湯化載方治療HZ，能明顯緩解HZ疼痛，促進(jìn)皰疹結(jié)痂、皮疹消退，降低帶狀皰后遺神經(jīng)痛發(fā)生率，值得推廣應(yīng)用。
