年輕患者腰椎間盤Pfirrmann分級(jí)與椎間盤膨出及突出的相關(guān)性研究

[摘要]目的 通過磁共振成像,觀察30歲以下腰痛患者腰椎間盤退行性改變的Pfirrmann分級(jí)特征以及相關(guān)的其他改變,并評(píng)價(jià)Pfirrmann分級(jí)與椎間盤膨出及突出的相關(guān)性。材料與方法 回顧性分析83例30歲以下因腰痛行腰椎MRI檢查的患者,其中男50例,女33例,平均年齡25歲。由2名影像科醫(yī)師對(duì)MR圖像進(jìn)行分析,觀察L1~S1椎間盤信號(hào)、椎間盤形態(tài)、纖維環(huán)撕裂、椎間盤膨出、椎間盤突出、椎體終板及許莫氏結(jié)節(jié)的改變。椎間盤退行性改變采用Pfirrmann分級(jí)。觀察指標(biāo)的相關(guān)性分析采用Pearson多元相關(guān)分析。結(jié)果 納入研究的椎間盤共415個(gè),其中Pfirrmann分級(jí)Ⅰ~Ⅱ級(jí)退變322個(gè)(77.6%),Ⅲ級(jí)退變56個(gè)(13.5%),Ⅳ級(jí)退變33個(gè)(7.9%),Ⅴ級(jí)退變4個(gè)(1.0%);103個(gè)椎間盤出現(xiàn)膨出或突出;椎間盤膨出以L4~5椎間盤最多(42.2%),且多合并Ⅲ級(jí)退變(51.1%);椎間盤突出以L5~S1椎間盤最多(51.7%),且多合并Ⅳ級(jí)退變(46.6%);17例患者出現(xiàn)椎間盤后方高信號(hào)區(qū)(HIZ),且均位于L4~5、L5~S1椎間盤水平;3例患者出現(xiàn)終板退變。多元相關(guān)分析結(jié)果顯示Pfirrmann Ⅲ級(jí)退變與椎間盤膨出有明顯相關(guān)性(r=0.972;P=0.006),與椎間盤突出無(wú)明顯相關(guān)性(r=0.639;P=0.245);Pfirrmann Ⅳ級(jí)退變與椎間盤突出(r=0.999,P=0.000)及HIZ (r=0.919,P=0.028)有明顯相關(guān)性,與椎間盤膨出無(wú)明顯相關(guān)性(r=0.769,P=0.128)。HIZ與椎間盤膨出(r=0.919,P=0.027)及突出(r=0.935,P=0.020)均有相關(guān)性。結(jié)論 腰椎MRI成像能夠清晰顯示年輕患者椎間盤早期退行性改變的不同病理階段;Pfirrmann Ⅲ退變與Pfirrmann Ⅳ級(jí)退變分別與椎間盤膨出和椎間盤突出有相關(guān)性;年輕患者應(yīng)及早發(fā)現(xiàn)椎間盤早期退變,以對(duì)臨床評(píng)價(jià)提供更有價(jià)值信息。

椎間盤退行性改變引起的下腰痛較常見,隨年齡的增長(zhǎng)和生活方式的改變,其發(fā)病率逐漸增高,也越來(lái)越年輕化。既往文獻(xiàn)中對(duì)老年人研究較多,而對(duì)年輕患者的研究較少。有文獻(xiàn)表明,30歲以前椎間盤即可發(fā)生退行性改變,而早期的椎間盤退變可以加速腰椎退變的進(jìn)程[1]。Pfirrmann分級(jí)可以較好的反應(yīng)椎間盤早期退變的過程,而椎間盤后方高信號(hào)區(qū)(high intensity zone,HIZ)通常提示纖維環(huán)撕裂,并與椎間盤膨出、突出形成下腰痛的主要原因。本研究以年輕人為對(duì)象,通過MRI觀察腰椎間盤早期退行性改變的影像學(xué)特征,同時(shí)評(píng)價(jià)椎間盤Pfirrmann分級(jí)與HIZ、椎間盤膨出和突出的相關(guān)性,以引起臨床對(duì)早期退行性改變的重視。

1 材料與方法

1.1 一般資料

回顧性分析2013年1月至2015年4月因下腰痛于我院行腰椎MRI檢查的患者,共91例。入選標(biāo)準(zhǔn):(1)患者因下腰痛就診;(2)年齡小于30歲;(3)既往無(wú)明確腰部疾病病史,包括先天性病變、腫瘤、骨折、感染及手術(shù)病史;(4)能夠配合完成MRI檢查。其中有8例因不符合入選標(biāo)準(zhǔn)排除在外(4例為腰椎壓縮骨折,1例為脊髓脊膜膨出,1例為腰椎結(jié)核、1例為術(shù)后蛛網(wǎng)膜粘連,1例為腰背部膿腫),最終入組83例,其中男50例,女33例,年齡16~30歲,平均年齡25歲。

1.2 檢查方法

采用GE Signa Excite 1.5 T超導(dǎo)磁共振掃描儀,掃描序列及參數(shù):矢狀面T2 FRFSE TR 2420 ms,TE 112 ms,層厚5 mm;矢狀面T1 FSE TR 460 ms,TE 11 ms 層厚5 mm;矢狀面FS T2 FRFSE TR 2740 ms,TE 102 ms,層厚5 mm;橫斷面T2 FRFSE TR 4520 ms,TE 110 ms。

1.3 影像評(píng)價(jià)

掃描所得圖像由2名有經(jīng)驗(yàn)的放射科醫(yī)師獨(dú)立閱片,觀察內(nèi)容包括以下方面:(1)椎間盤信號(hào)及形態(tài):在T2WI矢狀面觀察,椎間盤退變的分級(jí)采用Pfirrmann分級(jí)(Ⅰ~Ⅴ)(分級(jí)標(biāo)準(zhǔn)見表1)[2]。(2)椎間盤膨出及突出:椎間盤膨出定義為椎間盤在各個(gè)方向上大于椎體終板,向周圍均勻膨隆,無(wú)局限性改變;椎間盤突出定義為局限性椎間盤組織移位超出椎間隙范圍[3]。(3)椎間盤后方HIZ:結(jié)合T2WI及T2WI脂肪抑制序列矢狀面像、T2WI軸面像觀察,表現(xiàn)為椎間盤后方條狀高信號(hào)。(4)椎體終板:按Modic等[4]得終板標(biāo)準(zhǔn)進(jìn)行分型,正常:椎體終板信號(hào)正常,無(wú)改變;Ⅰ型:T1WI終板及鄰近骨質(zhì)為低信號(hào),T2WI相對(duì)正常終板為高信號(hào);Ⅱ型:T1WI比正常骨髓信號(hào)高,T2WI信號(hào)也升高,但不如T1 WI明顯;Ⅲ型:T1 WI、T2WI信號(hào)均降低。(5)許莫氏結(jié)節(jié):在T1WI及T2WI矢狀面像觀察。(6)其他發(fā)現(xiàn)病變結(jié)合圖像做相應(yīng)記錄。

2名醫(yī)師記錄結(jié)果一致的作為最終結(jié)果,不一致的由2名醫(yī)師討論,確定最終結(jié)果。

1.4 統(tǒng)計(jì)學(xué)處理

使用SPSS 22.0統(tǒng)計(jì)分析軟件,采用Pearson多元相關(guān)方法分析腰椎不同退行性改變之間的相關(guān)性,以P<0.05為差異有統(tǒng)計(jì)學(xué)意義。

2 結(jié)果

2.1 影像學(xué)觀察結(jié)果

83例患者,共415個(gè)椎間盤納入最終結(jié)果。其中Pfirrmann分級(jí)Ⅰ~Ⅱ級(jí)退變322個(gè)(77.6%),Ⅲ級(jí)退變56個(gè)(13.5%),Ⅳ級(jí)退變33個(gè)(7.9%),Ⅴ級(jí)退變4個(gè)(1%);103個(gè)椎間盤出現(xiàn)膨出或突出;椎間盤膨出以L4~5椎間盤最多(42.2%),且多合并Ⅲ級(jí)退變(51.1%);椎間盤突出以L5~S1椎間盤最多(51.7%),且多合并Ⅳ級(jí)退變(46.6%); 17例患者出現(xiàn)HIZ,且均位于L4~5、L5~S1椎間盤水平;3例患者出現(xiàn)終板退變,位于L4、L5、S1椎體;12個(gè)椎體出現(xiàn)許莫氏結(jié)節(jié),其腰椎各個(gè)椎體均有分布;2例患者出現(xiàn)腰椎滑脫。Pfirrmann分級(jí)、椎間盤改變及HIZ具體結(jié)果見表2,其影像學(xué)表現(xiàn)見圖1~3,許莫氏結(jié)節(jié)觀察結(jié)果見表3,椎體終板的觀察結(jié)果見表4。

2.2 Pfirrmann分級(jí)、椎間盤膨出、椎間盤突出及HIZ的相關(guān)性分析

將Pfirrmann分級(jí)、椎間盤膨出、椎間盤突出及HIZ在L1/2~L5/S1五個(gè)椎間盤節(jié)段分別所累及的椎間盤數(shù)量作為觀察指標(biāo)做多元相關(guān)分析,分析結(jié)果顯示:Pfirrmann Ⅲ級(jí)退變與椎間盤膨出有明顯相關(guān)性(r=0.972,P=0.006),而與椎間盤突出無(wú)明顯相關(guān)性(r=0.639,P =0.245);Pfirrmann Ⅳ級(jí)退變與椎間盤突出(r=0.999,P=0.000)及HIZ (r=0.919,P=0.028)有明顯相關(guān)性,而與椎間盤膨出無(wú)明顯相關(guān)性(r=0.769,P=0.128)。HIZ與椎間盤膨出(r=0.919,P=0.027)及突出(r=0.935,P=0.020)均有相關(guān)性。具體結(jié)果見圖4。

3 討論

兒童及青少年下腰痛的發(fā)病率隨年齡增長(zhǎng)逐步上升。目前國(guó)內(nèi)外文獻(xiàn)對(duì)椎間盤退行性改變的關(guān)注對(duì)象多為老年人,而對(duì)年輕患者關(guān)注較少。Zitting等[5]對(duì)青少年椎間盤突出的流行病學(xué)調(diào)查表明:對(duì)12 058名新生兒做隨訪觀察,結(jié)果顯示,15歲以前無(wú)一名觀察對(duì)象因椎間盤突出住院,年齡增加至20歲時(shí)數(shù)量增加至0.1%~0.2%,有明顯上升趨勢(shì);到28歲時(shí),9.5%的男性和4.2%的女性因椎間盤突出入院。所以,年輕人腰椎退行性改變應(yīng)當(dāng)引起重視。諸多研究表明,現(xiàn)有的基礎(chǔ)病變(微小病變,早期退行性改變等)會(huì)加速椎間盤退行性改變的進(jìn)展[6-7]。所以及早發(fā)現(xiàn)椎間盤早期的退行性改變,或許對(duì)疾病的發(fā)展及預(yù)后有積極作用。因此,本研究以30歲以下人群作為研究對(duì)象,通過MRI成像觀察腰椎早期退行性改變的不同方面,并探討椎間盤早期的退行性改變與椎間盤膨出及突出的關(guān)系。

3.1 髓核退變與椎間盤膨出、突出的關(guān)系

髓核的退行性改變與腰痛有密切關(guān)系。髓核組織中含有大量蛋白聚糖與Ⅱ型膠原結(jié)合形成的網(wǎng)眼狀結(jié)構(gòu),其內(nèi)可儲(chǔ)存大量水分子,以維持髓核內(nèi)水合離子濃度的穩(wěn)定性,保持髓核的凝膠狀態(tài)及彈性,為椎間盤提供抵抗壓縮的載荷能力[8]。所以,髓核的退行性改變通常發(fā)生較早。MRI以獨(dú)特的成像優(yōu)勢(shì),通過髓核內(nèi)部水分的改變,可以清晰顯示退行性變化的動(dòng)態(tài)過程。目前存在多個(gè)以MRI為基礎(chǔ)的椎間盤退變分級(jí)系統(tǒng),其中Pfirrmann評(píng)分系統(tǒng)作為臨床及科研被廣泛接受[2-9],它可以較好的反應(yīng)椎間盤早期的退行性改變。我們通過對(duì)415個(gè)椎間盤的觀察發(fā)現(xiàn),椎間盤處于Ⅰ~Ⅱ級(jí)退變者占77.6%,且L1/2~L3/4椎間盤占多數(shù),提示年輕患者L1/2~L3/4節(jié)段椎間盤相對(duì)處于基本正常或水分剛剛開始丟失階段;Ⅲ~Ⅳ級(jí)退變占21.4%,且大部分(69.9%)發(fā)生于L4~5、L5~S1椎間盤,提示髓核內(nèi)水分丟失嚴(yán)重的區(qū)域位于L4~5、L5~S1椎間盤節(jié)段,部分髓核開始出現(xiàn)壓縮改變;Ⅴ級(jí)退變少見,提示嚴(yán)重的髓核退變發(fā)生較少。因此,對(duì)于年輕患者而言,大部分椎間盤處于相對(duì)正常狀態(tài),退變相對(duì)嚴(yán)重的區(qū)域位于L4~5、L5~S1椎間盤,并處于Ⅲ~Ⅳ級(jí)退變這一關(guān)鍵時(shí)期。從中我們推測(cè)椎間盤早期退變始于L4~5、L5~S1椎間盤,并且年輕患者多處于退變的進(jìn)展中,而未達(dá)到晚期的退變。

本組納入研究的患者中,椎間盤膨出或突出的患者占有較高比例(62/89),椎間盤膨出和突出的部位多位于L4~5、L5~S1椎間盤,且椎間盤膨出以Ⅲ級(jí)退變比例最高,椎間盤突出以Ⅳ級(jí)退變比例最高。統(tǒng)計(jì)結(jié)果也表明,Ⅲ級(jí)退變與椎間盤膨出有明顯相關(guān)性,Ⅳ級(jí)退變與椎間盤突出有明顯相關(guān)性。所以我們推測(cè)PfirrmannⅢ級(jí)、Ⅳ級(jí)退變?cè)谧甸g盤整體退變的發(fā)展中起關(guān)鍵作用,椎間盤Ⅲ~Ⅳ級(jí)退變提示髓核內(nèi)水分丟失嚴(yán)重,椎間盤高度開始減低,提示髓核被壓縮,而使其內(nèi)壓增大,容易使椎間盤膨出。但本研究中并非所有突出及膨出的椎間盤處于Ⅲ、Ⅳ級(jí)退變,部分為Ⅱ級(jí)退變,說(shuō)明在髓核水分剛開始丟失時(shí)即出現(xiàn)膨出或突出,這可能與年輕患者不適當(dāng)?shù)倪\(yùn)動(dòng)或外傷有關(guān)。

3.2 HIZ與椎間盤膨出、突出的關(guān)系

HIZ首先由Aprill等[10]提出并定義,即MRI矢狀面T2WI顯示腰椎間盤后方小而圓的局限性高信號(hào)區(qū)。王子軒等[11]報(bào)道,椎間盤后方HIZ發(fā)病率約占31.7%,但其研究的對(duì)象以中老年患者為主。本研究83例年輕患者中,有17例出現(xiàn)HIZ,約占20.4%,表明年輕患者也有相當(dāng)比例出現(xiàn)HIZ。Peng等[12]通過研究發(fā)現(xiàn)HIZ區(qū)域中正常組織被紊亂的富含毛細(xì)血管的肉芽組織取代,包括纖維母細(xì)胞及新生血管,并逐漸向外擴(kuò)大,證明這是機(jī)體對(duì)破裂纖維環(huán)的修復(fù)作用。諸多研究證實(shí)HIZ是診斷椎間盤原性下腰痛的重要MRI征象。Olmarker[13]研究發(fā)現(xiàn),疼痛誘發(fā)試驗(yàn)和疼痛阻止試驗(yàn)可以證實(shí),纖維環(huán)的撕裂可以引起強(qiáng)烈的疼痛。這些疼痛主要?dú)w因于破壞的組織和炎性組織對(duì)敏感神經(jīng)的刺激,移位的髓核內(nèi)部的化學(xué)物質(zhì)向周圍彌散,可以加劇疼痛。Videman等[14]研究發(fā)現(xiàn)纖維環(huán)撕裂常發(fā)生于椎間盤退變的早期,并且與髓核的快速退變有關(guān)。Schmidt等[15]數(shù)學(xué)模型表明,未完全發(fā)生退行性改變的髓核富含水分,其內(nèi)水壓增高,纖維環(huán)在緊張的情況下容易發(fā)生破裂。本研究中HIZ出現(xiàn)的部位均位于L4~5、L5~S1椎間盤,并且Pfirrmann分級(jí)均為Ⅲ~Ⅳ級(jí),并且HIZ與椎間盤膨出、椎間盤突出均有明顯相關(guān)性,因此,椎間盤膨出、突出、纖維環(huán)撕裂與椎間盤Ⅲ~Ⅳ級(jí)退變密切相關(guān)。椎間盤Ⅲ~Ⅳ級(jí)退變提示髓核內(nèi)水分丟失嚴(yán)重,椎間盤高度開始減低,提示髓核被壓縮,而使其內(nèi)壓增大,同時(shí)椎間盤高度的變化,使垂直壓力作用于纖維環(huán)的局部,而不是均勻的分布于纖維環(huán)周圍,最終導(dǎo)致纖維環(huán)局部破裂,而纖維環(huán)的破口,又給髓核突出提供通道,最終導(dǎo)致一系列事件發(fā)生。所以,髓核內(nèi)部水分的丟失可能為始動(dòng)因素。Carragee等[16]研究也表明,不管是人類還是動(dòng)物,如果在纖維環(huán)上人為的制造裂隙,這會(huì)導(dǎo)致加速細(xì)胞介導(dǎo)的退行性改變過程。說(shuō)明,早期發(fā)現(xiàn)纖維環(huán)撕裂并及時(shí)干預(yù)治療,對(duì)腰椎退行性改變的發(fā)展的控制有積極意義。

圖4 以L1~S1五個(gè)椎間盤階段的數(shù)量為觀察對(duì)象,結(jié)果顯示:Pfirrmann Ⅲ退變與椎間盤膨出有明顯相關(guān)性;Pfirrmann Ⅳ級(jí)退變與椎間盤突出及HIZ有明顯相關(guān)性。HIZ與椎間盤膨出及突出均有相關(guān)性Fig. 4 The curve diagram shows the pearson correlation between different aspects associated with disc degeneration. 3.3 終板退變

本研究中83例患者僅有3例(3.6%)出現(xiàn)終板退行性改變,而文獻(xiàn)中張文煜等[ 1 7 ]研究中納入1681例患者,有432例患者出現(xiàn)終板退變,其發(fā)生率約為25.6%,本研究明顯低于張的研究。說(shuō)明,年輕患者終板退行性改變發(fā)生率較低。本研究中3例終板退變僅為早期的Ⅰ型和Ⅱ型退變,無(wú)Ⅲ型退變,且均發(fā)生于椎間盤膨出或突出的椎體層面,相應(yīng)椎間盤退變?yōu)棰蟆跫?jí),因此推測(cè)椎體終板退行性改變可能為相對(duì)晚期的改變。

3.4 許莫氏結(jié)節(jié)

本研究中5例出現(xiàn)許莫氏結(jié)節(jié),累及12個(gè)椎體,其出現(xiàn)的部位在各個(gè)椎體都有發(fā)生,并且與L4~5、L5~S1膨出及突出不太相符,似乎與椎間盤Pfirrmann分級(jí)之間也無(wú)明顯規(guī)律可循。潘希敏等[18]研究發(fā)現(xiàn)許莫氏結(jié)節(jié)廣泛存在于各年齡組、各退變級(jí)別的腰椎間盤中,退變級(jí)別高的腰椎間盤并不意味著容易發(fā)生許莫氏結(jié)節(jié),相反許莫氏結(jié)節(jié)的出現(xiàn)會(huì)加速間盤的退變。

綜上所述,對(duì)于下腰痛的年輕患者,MRI可以清晰顯示腰椎間盤退變的早期特征性改變,椎間盤Pfirrmann Ⅲ~Ⅳ退變?yōu)殛P(guān)鍵階段,可能為椎間盤膨出或椎間盤突出的前期改變,同時(shí)椎間盤后方高信號(hào)的出現(xiàn)也可能會(huì)加速椎間盤膨出或突出的進(jìn)展,在臨床工作中應(yīng)當(dāng)及早發(fā)現(xiàn)并引起重視。

MR Imaging assessment of correlations between Pfirrmann’s grading system and lumbar disc bulge and lumbar disc protrution in young patients

AbstractObjective: To find the MRI characteristics of the lumbar intervertebral disc degeneration in patients less than 30 years old with low back pain, and try to evaluate the correlations between Pfirrmann’s grading scale and intervertebral disc bugle and intervertebral disc protrution. Material and Methods: It is a cross-sectional and observational study. A total of 83 patients with low back pain performed lumbar MRI examination. All patients were less than 30 years old, of whom 50 were men and 33 were women, with mean age 25. All MR imaging were evaluated by two radiologists in the following aspects: signal intensity and morphology of the intervertebral discs, annular tear, intervertebral disc bulge, intervertebral disc protution, Modic changes and the Schmorl’s nodes. The intervertebral discs degeneration were evaluated through Pfirrmann’s classification system. The association between these different aspects were analyzed by Pearson multiple correlation. Results: A total of 415 intervertebral discs were examined, of which gradeⅠ—Ⅱ discs were 322 (77.6%), grade Ⅲ 56 (13.5%), grade Ⅳ 33 (7.9%) and grade Ⅴ 4 (1%). One hundred and three intervertebral discs were found with disc bulge and disc protrution, most of which were at disc L4—5 and L5—S1 level and werebook=204,ebook=51common in grade Ⅲ and grade Ⅳ degenerations. Seventeen patients were found with HIZ, and were all at disc L4—5 and L5—S1 level. Three patients were found with modic change. The statistic results show that grade Ⅲ degeneration were correlated well with disc bulge (r=0.972, P=0.006), but not with disc protrution (r=0.639, P=0.245) and the grade Ⅳ disc degeneration were correlated with disc protrution (r=0.999, P=0.000) and HIZ (r=0.919, P=0.028), but not with disc bulge (r=0.769, P=0.128). HIZ were correlated well with both disc bulge (r=0.919, P=0.027) and disc protrution (r=0.935, P=0.020). Conclusions: MRI is a good imaging modality for detecting the pathology character of disc degeneration in early stage. Grade and grade degenerations have close relationship with intervertebral disc bulge and protrution. It may have great clinical value to detect early changes of intervertebral disc degeneration in young patients.
